What does a TSH level of 60 mean for my thyroid?

I recently had my thyroid tests done, and my TSH level is around 60. I have been taking an ayurvedic medicine (Thyrawin). I would like proper medical guidance, medication if required, and advice on further tests and management.

I have read your query and can understand your concern.

Your thyroid level, especially TSH (thyroid-stimulating hormone), is 60, which is very high, and you are taking ayurvedic herbal medicine; your current weight shows obesity, which shows signs of hypothyroidism (decreased levels of thyroid hormone).

The signs and symptoms of hypothyroidism are weight gain, tiredness, cold intolerance, dry skin, constipation, menstrual irregularities, and so on.

So I need to know if you have done these lab investigations recently, within two months:

  1. Thyroid profile test for TSH (thyroid-stimulating hormone), free T3 (free triiodothyronine), and T4 (thyroxine).

  2. Ultrasound of the neck to see the status of the gland.

  3. Anti-TPO antibodies (anti-thyroid peroxidase antibody, a blood test that detects antibodies attacking the thyroid peroxidase (TPO) enzyme).

  4. Fasting blood glucose level/Hb1Ac (glycated hemoglobin).

  5. Lipid profile.

Right now, you are not taking proper medication for hypothyroidism; you need to start taking the tablet. Levothyroxine 75 mcg one tab once daily on an empty stomach in the morning, and wait for 30 minutes after that to take food.

You need to follow up after six weeks to check the levels of thyroid profile to see if the medication dosage needs to be increased or not.

As your BMI (body mass index) is 37, you fall into class 2 obesity, also called moderate obesity, which is defined by a body mass index (BMI) of 35 to less than 40 kg/m², so you need to start taking a balanced diet with less carbs, and weight reduction by exercising.
