Hi doctor,

I am a 26-year-old male. I started to have symptoms three months ago. I have mild hand tremors, increased eye floaters, and a tingling sensation in my left hand. I took HIV, HCV, HBV, and HAV, and all the test results were negative. Kindly suggest tests to diagnose my health issue.

First of all, a physical examination should be done along with a neurological and cranial nerve examination. For tremors and tingling sensations, the following investigation should be done. They are serum B12 level to rule out megaloblastic anemia, CBC (complete blood count) with peripheral smear examination, serum TSH (thyroid-stimulating hormone), free T4 (thyroxine) estimation to rule out hyperthyroidism, serum calcium level to rule out hypoglycemia, serum electrolyte (includes sodium and potassium), and PPBS (postprandial blood sugar) sugar estimation to rule out diabetic neuropathy, which can be suspected strongly. After examination, if it seems to be a spinal or neurological cause, then a CT (computed tomography) scan can be done.

For tingling, try to rule out vitamin deficiency and other causes. If there is no clear cause, then nerve conduction velocity (NCV) testing can be done as further workup. According to the cause, further specific treatment can be given. For example, if hypoglycemia is present, then a calcium tablet or cholecalciferol sachet with milk can be prescribed. If B12 deficiency is present, then Cyanocobalamin injections are prescribed. Consult a physician for examination while keeping all this in mind.
