Can women at 44 with TNBC have children after taking Lupron?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hello doctor,

I went through aggressive treatment for triple-negative breast cancer, including chemotherapy, and Lupron was used during treatment. My periods have still not fully returned yet. My questions are:

  • Can someone at 44 with triple-negative breast cancer still have children after Lupron treatment?

  • In breast cancer patients at this age, how often does ovarian function recover after chemotherapy and Lupron treatment?

  • Should fertility testing be done now, or is it better to wait longer?

  • How do doctors usually assess fertility potential after cancer treatment at this stage?

I am trying to balance hope with being realistic and not setting myself up for heartbreak. It feels unfair that survival questions suddenly turn into fertility deadlines overnight.

Kindly help.

Hello,

Welcome to icliniq.com.

I read your query and understood your concern.

Leuprolide (Lupron) is often used during chemotherapy to help protect ovarian function, so some women are still able to have children afterward. However, at the age of 44, fertility naturally declines, and chemotherapy for triple-negative breast cancer can further reduce ovarian reserve and ovarian function.

Not having your period return yet does not automatically mean that pregnancy is impossible. In some cases, ovarian function can recover months or even more than a year after treatment. However, in women in their 40s, there is also a higher chance that menopause may become permanent after chemotherapy.

Doctors can perform fertility testing now rather than waiting longer. Blood tests such as AMH (anti-mullerian hormone), FSH (follicle-stimulating hormone), and other reproductive hormone levels, along with a pelvic ultrasound, can help assess whether the ovaries are recovering and what fertility options may still be possible.

Because your cancer was triple-negative breast cancer, which is not hormone-driven, pregnancy after treatment is often considered safer than in some other breast cancer subtypes once your oncology team confirms that it is medically appropriate.

The most important next step is meeting with both your oncologist and a fertility specialist to discuss your situation realistically and explore possible options early. Wanting to hold on to hope while also preparing yourself emotionally for uncertainty is a very understandable feeling after everything you have been through.

I hope you are satisfied with my answer. For further queries, you can consult me at iCliniq.

Thank you.
