I am a 64-year-old man with a 15-year history of type 2 diabetes, complicated by chronic kidney disease (eGFR 45 mL/min) and retinopathy. My recent HbA1c is 7.9%, and I am currently on a regimen of Metformin (500 mg daily) and Dapagliflozin (10 mg daily). Over the past few months, I have been experiencing numbness and tingling in my feet, which I suspect might be diabetic neuropathy. My fasting glucose is 160 mg/dL, and my blood pressure is well-controlled at 130/80 mmHg with Losartan.
My doctor mentioned the possibility of adding medications like GLP-1 receptor agonists, but I am concerned about the cost and potential side effects. Is there a way to optimize my current treatment plan to prevent further complications? What can be done to address the neuropathy symptoms, and should I be more concerned about the progression of my kidney disease?
