What could be the cause of fever, chills, vomiting, stomach and body aches?

I have had a fever for the past seven days. I have also had chills, stomach aches, vomiting, body aches, heaviness in the stomach, and loss of appetite.

I went through your reports (attachments removed to protect the patient's identity).

1. Complete blood count (CBC): Hemoglobin and total leukocyte count (TLC) are normal.

2. The mean corpuscular volume (MCV) is high because of low vitamin B12 levels.

3. Kidney function test (KFT): Normal.

4. Liver function test (LFT): Normal except for mild elevation of serum glutamic-oxaloacetic transaminase (SGOT), serum glutamic pyruvic transaminase (SGPT), and alkaline phosphatase (ALP) levels.

At this level of enzyme values, there is nothing to worry about. These values are mildly high because enteric fever is the most likely cause of them.

5. Typhidot is positive for IgM (immunoglobulin M), which means acute typhoid infection. I think this is the reason for the fever, vomiting, and stomachache.

6. Low vitamin B12 and vitamin D levels, and it needs treatment.

7. Urine routine and microscopy are normal.

I would suggest the following:

1. Take a tablet of Paracetamol (analgesic) 500 mg (milligrams), thrice daily after food.

2. Take a tablet of Azithromycin (macrolide antibiotic) 500 mg, once daily after food.

3. Take a tablet of Cefixime (Cephalosporin antibiotic) 200 mg, twice daily after food.

4. Tab Emeset (Ondansetron) 4 mg, SOS (as needed) for vomiting.

5. Take a tablet of Pantoprazole (antacid) 40 mg, once daily before food.

6. Take a soft diet warm homemade cooked food.

I hope you are not allergic to the above medications, which are for five days.

For typhoid fever, the best antibiotic is Ceftriaxone (Cephalosporin antibiotic), which is given in intravenous (IV) form. You take the above-suggested antibiotics or Ceftriaxone (available in IV form) to treat typhoid fever. If possible, you should visit the nearest healthcare facility for an injection.

For vitamin deficiency, you need to take an injection of vitamin B12 and a tablet form of vitamin D.

Kindly consult a specialist, talk with them, and take medications with their consent.
