This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hello doctor,

I have an ulcerated area that has been present for about two weeks. It appears to be infected, and the redness has spread to the surrounding skin. It is extremely painful, even with slight movement.

What should be done?

Kindly advise.

Thank you.

Hello,

Welcome to icliniq.com.

I have read your query and understand your concern.

From the picture (attachment removed to protect the patient’s identity) you have shared, the lesion appears to be a localized skin ulcer with surrounding inflammation, most likely due to a secondary bacterial infection. The redness spreading into the surrounding skin suggests early cellulitis, which is why you have severe pain.

The most likely possibilities include an infected insect bite, a localized skin abscess, or ecthyma (a deeper skin infection caused by bacteria such as Staphylococcus or Streptococcus).

Increasing redness around the wound with severe pain even with movement and ulcer formation indicates that the infection is progressing. I urge you to seek an in-person medical evaluation soon, as antibiotics will be required.

I would suggest the following measures:

  1. Keep the wound clean, wash gently with soap and water, and apply a topical antibiotic like Mupirocin ointment.

  2. Warm compresses for about 10 minutes, thrice daily.

  3. Take Acetaminophen or Ibuprofen for pain.

  4. Meanwhile, you can start the antibiotic Augmentin 625mg (Amoxicillin and Clavulanate potassium) twice a day with Doxycycline 100mg twice a day.

Also, let me know if this lesion started as a bite, pimple, blister, or scratch before becoming an ulcer. Is there any pus discharge and associated fever?

I hope it helps.

For further queries, you can consult me.

Thank you.

Patient's Query

Hello doctor,

Thank you for the reply.

Can you send me a script to get that medication, please?

And no, it started as a burn; I burnt myself with a lighter. Yes, I have had it for about two or three weeks now. Need something to help make it better.

Kindly advise.

Thank you.
