How do I, 23, manage low weight and chest pain?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hello doctor,

I am a 23-year-old male who works night shifts from 9:00 PM to 5:00 AM. 55 pounds, at a height of 167.6 centimetres. I have been experiencing chest pain for approximately six to seven months. However, my Electrocardiogram (ECG), Echocardiogram (Echo), blood tests, and cholesterol tests have all returned normal results. During this period, I also experienced dizziness, unusual chest sensations, and heart palpitations.

At present, I no longer feel dizzy during routine daily activities. However, I continue to experience dizziness and heart palpitations after playing futsal. I also notice dizziness after drinking milk tea. Please help.

Thank you.

Hello,

Welcome to icliniq.com

Thank you for providing a detailed history. Based on your symptoms, including chronic chest discomfort, dizziness, palpitations, worsening after physical exertion such as playing futsal, and episodes triggered by milk tea, together with a normal basic cardiac evaluation such as an Electrocardiogram and Echocardiogram, the overall pattern is more suggestive of a non-cardiac cause.

Several important factors should be carefully considered.

  1. Extremely low body weight: A body weight of approximately 55 pounds, at a height of 167.6 centimeters, corresponds to a body mass index of roughly 8.9. This is classified as severe underweight and may pose serious health risks. Such a low body mass index can lead to orthostatic hypotension, reduced cardiovascular reserve, hypoglycemia, and electrolyte imbalances. Any of these disturbances can contribute to dizziness, weakness, and palpitations.

  2. Sensitivity to caffeine and sugar: Experiencing dizziness after consuming milk tea may be related to the effects of caffeine and sugar. These substances can affect heart rate, blood pressure, and blood glucose levels. In individuals with very low body weight, even modest amounts of stimulants may produce exaggerated responses, including palpitations and lightheadedness.

  3. Exercise-related symptoms: Dizziness and palpitations after playing futsal may reflect reduced physical conditioning, limited physiological reserve, or autonomic instability. These problems are more likely in the presence of low body mass or nutritional deficiencies, such as iron deficiency, vitamin B12 deficiency, or electrolyte abnormalities.

  4. Sleep and circadian rhythm disruption: Working night shifts can disturb the normal circadian rhythm. Disruption of the sleep-wake cycle can affect sleep quality, hormone balance, and autonomic nervous system function. These changes may worsen symptoms such as fatigue, dizziness, and palpitations.

Several possible differential diagnoses for your symptoms.

  1. Severe underweight and nutritional deficiency states: Severe undernutrition may lead to electrolyte disturbances, hypoglycemia, anemia, vitamin deficiencies, and reduced muscle mass, all of which can produce dizziness and palpitations.

  2. Postural orthostatic tachycardia syndrome or orthostatic intolerance: These conditions are more common in underweight or dehydrated individuals and may present with dizziness, rapid heart rate, and palpitations, particularly after standing or exertion.

  3. Anxiety or panic-related conditions: In young individuals with normal cardiac investigations, anxiety-related factors may contribute to chest discomfort and palpitations. Symptoms may be aggravated by stress, sleep disturbance, and stimulants.

  4. Autonomic dysfunction associated with malnutrition: Malnutrition can affect the autonomic nervous system, which regulates heart rate and blood pressure, potentially leading to dizziness and palpitations.

Further evaluation may be helpful. Reasonable tests to consider include complete blood count, serum electrolytes (including sodium, potassium, magnesium, and calcium), thyroid function tests (including thyroid-stimulating hormone), fasting blood glucose and glycated hemoglobin, iron studies (including ferritin), and measurement of vitamin B12 and vitamin D levels. If palpitations continue, ambulatory electrocardiographic monitoring, such as a Holter monitor or an exercise stress test, may also be useful.

From a practical standpoint, it is very important to seek a medical consultation for a comprehensive nutritional and general health assessment. Improving nutritional status should be a priority. Limiting or avoiding stimulants such as milk tea, energy drinks, and excessive coffee may help reduce symptoms. Intense physical exertion should ideally be avoided until body weight and overall health improve, with a focus instead on gentle and gradual physical activity. Ensuring adequate sleep, maintaining proper hydration, and having appropriate salt intake may also be beneficial. If anxiety or stress appears to be contributing, a mental health evaluation may also be considered.

Overall, your symptoms may be related to the physiological effects of severe underweight status, possibly combined with autonomic imbalance or nutritional deficiencies. A thorough evaluation by a qualified healthcare professional is essential for accurate diagnosis and appropriate management.

I hope this information is helpful. Please do not hesitate to ask if you have any further questions. I would be happy to assist you.

Thank you.
