Why is there a clear watery fluid leak after ejaculation?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hello doctor,

Clear, watery discharge occurs after the ejaculation of a normal ejaculate. The discharge is not discolored and has no odor. Tests for STDs, UTIs, prostatitis, and balanitis have all come back negative.

What could be causing this clear, watery, non-smelly discharge that appears within a minute after ejaculation and continues for about five to ten minutes? It is not precum, as there is no leakage before ejaculation, only after. The discharge happens exclusively post-ejaculation and lasts for several minutes.

Ejaculation occurs normally, followed by continuous watery discharge for up to 10 minutes. It is also impossible to get an erection again during this time without using Viagra or similar medications. Why does this clear discharge appear after ejaculation, even when not erect?

Kindly advise.

Hello,

Welcome to icliniq.com.

I understand your concern.

Based on your description, the clear, watery discharge you notice after ejaculation, lasting several minutes and occurring only post-ejaculation, is most likely due to residual seminal or prostatic fluid leakage rather than an infection or disease.

After orgasm, the muscles of the pelvic floor and urethra gradually relax, and a small amount of remaining fluid (which can mix with prostatic secretions and urine) may continue to flow out as the body completes its natural clearing process.

This is sometimes called post-ejaculatory urethral drainage and is generally harmless.

It tends to occur more often in men over 40, those with mild pelvic floor muscle weakness, previous inflammation, or reduced muscle tone from medications such as Sertraline (selective serotonin reuptake inhibitor), which can slightly affect ejaculation control.

If there is no pain, burning, odor, discoloration, or urinary problem, this type of discharge usually does not indicate infection, prostatitis, or retrograde ejaculation. Strengthening the pelvic floor through Kegel exercises can sometimes reduce the duration of post-ejaculatory leakage by improving muscle control around the urethra.

Staying well-hydrated and emptying the bladder after ejaculation can also help flush out any remaining fluid. However, if there is any change in the discharge’s appearance, development of a new odor, persistent discomfort, or difficulty with erections or urination, it is advisable to have a repeat evaluation by a urologist.

In most cases, though, what you are describing is a benign physiological process rather than a sign of disease.

I hope you are satisfied with my answer. For further queries, you can consult me at iCliniq.

Thank you.
