Hello doctor,

My 68-year-old father had bladder cancer surgery two years ago and now has metastatic urothelial carcinoma to the lungs. He has been on immunotherapy for six months, but lately has been extremely fatigued and short of breath, which worries us about disease progression versus side effects.

His last CT scan showed a mixed response. Are there other systemic treatments if immunotherapy stops working, and how do we know when to switch?

These symptoms can understandably raise concerns about disease progression or treatment-related side effects, and careful evaluation is essential.

Your father has metastatic urothelial carcinoma with lung involvement and has been on immunotherapy for six months. A mixed response on CT (computed tomography) scan, where some lesions shrink while others remain stable or progress, is not uncommon during immunotherapy.

Fatigue and shortness of breath may be related to treatment side effects, anemia, infection, or pulmonary complications, including progression of lung metastases. These symptoms should be promptly discussed with his oncologist, as they may influence further treatment decisions.

The probable cause of his current symptoms is lung metastasis. However, contributing factors such as anemia, infection, or immune-related lung toxicity must also be considered.

Recommended investigations include imaging studies such as CT scans or positron emission tomography scans to assess tumor progression or new lesions.

Additional tests include complete blood counts, kidney and liver function tests, chest X-ray, electrocardiogram, and pleural fluid analysis if effusion is present.

The differential diagnosis includes primary lung cancer, pulmonary tuberculosis, lung abscess, and fungal cavitary lesions. These possibilities should be ruled out based on imaging, laboratory findings, and clinical correlation.

The probable diagnosis remains lung metastasis from urothelial carcinoma. This is supported by the known metastatic disease and respiratory symptoms.

The treatment plan may involve alternative systemic therapies such as platinum-based chemotherapy with Cisplatin or Carboplatin.

Other options include single-agent chemotherapy (Paclitaxel, Docetaxel, or Vinflunine), targeted antibody–drug conjugates such as Enfortumab Vedotin or Sacituzumab Govitecan, and enrollment in clinical trials if available.

Symptomatic management includes treatment of acute infections, use of bronchodilators, corticosteroids if indicated, antibiotics when infection is suspected, and long-term oxygen therapy if required. Supportive care should be tailored to symptom severity and overall functional status.

Preventive and supportive measures focus on controlling fatigue, shortness of breath, and pain, along with adequate nutritional and hydration support.

Close monitoring for immunotherapy-related side effects involving the lungs, liver, thyroid, or other organs is essential, with oxygen therapy provided when necessary.
