Hello doctor,

I am 19 years old, 105 pounds, and 5 feet and 3 inches tall. For the past week, I have been experiencing abnormal vaginal discharge. It is brown colored, clots up, and looks black. It has been heavier the past few days which is making me curious. I have no discomfort. My last PMS was about two weeks ago. The last time I was tested for STDs was about one week ago, and the last time I had unprotected sex was about four days ago. I have an IUD.

I can understand your concern.

From your history, I suspect a vaginal infection. This need not be the significant STD (sexually transmitted disease) that you have tested. This vaginal infection (a few bacteria , viruses and fungi) may be transmitted from the semen. It move up into the cervical canal and then into the uterus. The infection from semen enters the vagina and cervical canal. It ascends into the uterus and then into the tubes and the sides of the uterus and surrounding structure (muscles and ligaments).

So there may be clear or whitish discharge, becomes yellowish or greenish or grayish or dirty brownish, with or without foul smell and itching, throbbing sensation in the lower abdomen before periods, mild fever during periods, pain becomes severe during periods and while doing sex, especially at the time of orgasm, when traction occurs on the surrounding muscles and ligaments. Pain radiate to thighs, lower back and bone above vaginal opening. Sex and periods followed by excessive discharge for a few more days.

There will be raw areas over the cervix, which remain raw and each time when semen or the penis touches it, you get fresh red or pink blood with a lot of discharge.

This infection gradually goes into the uterus and increases the thickness of the endometrium, Inner lining of the uterus, so the thickness is not uniform and not healthy. This layer keeps peeling off as thick, brownish, or purple-colored clots or fleshy masses, which keep on falling as the whole thickness has not gone. As time passes the infection may not be evident in tests, but the bleeding from the uterus is increased. Before treatment confirm that you are not pregnant. I would suggest you and your partner can take:

1. Tablet Fluconazole 150 mg one tablet after lunch.

2. Tablet Azithromycin 1 gm one tablet, one hour before food.

3. Tablet Secnidazole forte two tablets after dinner.

These should be taken on the same day by both of you. Later you have capsule Doxycycline 100 mg twice a day for one week, vaginal wash or any cleanser twice a day and vaginal pessary. Capsule Clingen (Clindamycin and Clotrimazole) forte or Candid -CL or Cansoft CL once at night for five nights.

Your partner can have local application of any antibiotic and antifungal cream. Candid B or Candiderma or Lobate GM on the penis twice a day after washing for seven days. Do not have sex till the infection is fully gone, for around two-three weeks. Do treat it as delay may send the infection inwards and may affect both you and your partner's future fertility. If the infection is not relieved after all treatment, re-evaluate and think about the removal of the IUD (intra uterine device), as this may be harboring infection.
