Patient's Query
Hello doctor,
My period cycle is 31 days. My last period was 28 days ago, and it lasted for 29 to 25 days ago for four days. I had sexual intercourse 24 and 23 days ago and again 16 days ago. I took iPill (Levonorgestrel) as a contraceptive, 14 days ago, and again five days ago.
Now I am experiencing a heavy white discharge that is thicker than usual and has a foul odor. Does it indicate pregnancy, and why is this occurring? My expected period date is in five days. What can I do now as a remedy? The white discharge is thick, milky, and clumpy with a foul odor. Sometimes I experience itching. Why does this occur?
Kindly help.
Thank you.
Hello,
Welcome to icliniq.com.
I read your query and understood your concern.
I will break this down for you. You said your cycle length is usually 31 days, your last period was 28 days ago, and it lasted four days. You had unprotected intercourse 24 days ago, 23 days ago, and 16 days ago. You took an emergency contraceptive pill 14 days ago and again five days ago.
Now you are noticing a thick white discharge with foul smell and itching. You are worried whether this means pregnancy or some infection. Let me first tell you: the discharge you are describing, thick, clumpy, white like curd or milk, sometimes itchy and with a smell, is not a typical sign of pregnancy.
Pregnancy does not present like this. Early pregnancy symptoms are usually missed period, breast heaviness, nausea, and sometimes fatigue. White, foul‑smelling, clumpy discharge with itching is very typical of vaginal infection, mostly yeast (Candida) or mixed infection.
Also, regarding pregnancy risk, you had sex around day four and day five after your period, and again on day 12. In a 31‑day cycle, ovulation usually occurs around day 16 or 17. So the intercourse 16 days ago has the maximum chance of coinciding with the fertile window.
But you did take an emergency contraceptive pill (ECP) twice in the same cycle. ECP is effective if taken within 72 hours, but repeated use in the same cycle can disturb your hormones, delay your period, or make it early.
So the period expected within five days may come late or early. You cannot take ECP repeatedly like this because it reduces effectiveness and disrupts your cycle.
Now, about the discharge: this sounds like an infection, not pregnancy. I would think about vaginal candidiasis (yeast infection), possibly mixed with bacterial vaginosis. The itching you describe fits yeast. A foul smell usually indicates bacterial vaginosis.
Many times, both coexist. So the reason is not the ECP itself directly, but after hormonal fluctuations and stress, vaginal flora balance can get disturbed, leading to infections. Also, intercourse itself can predispose to infections if hygiene is not perfect.
What to do now:
You need a local examination, ideally by a gynecologist, but a simple vaginal swab or wet mount will confirm whether it is yeast or bacterial. Based on that, proper antifungal or antibacterial treatment will be given.
Right now, you can try some home supportive measures like keeping the area dry, avoiding tight undergarments, using cotton underwear, and washing only with plain water (avoid too much soap).
But to treat properly, you will need antifungal medicines (oral Fluconazole or vaginal pessary like Clotrimazole), and sometimes an antibiotic like Metronidazole if bacterial vaginosis is present. These cannot be self‑started without a doctor's advice because dose and duration matter.
Pregnancy-wise, the only way to know is to do a urine pregnancy test in five days if your period does not come. Right now, it is too early. The discharge does not mean pregnancy. Do not confuse the. I will outline the points clearly below.
Probable cause can be:
Repeated intake of ECP in one cycle leads to hormonal imbalance and disturbance of vaginal flora.
Vaginal yeast infection (Candida), possibly mixed with bacterial vaginosis.
Sexual activity may have contributed to altering the normal vaginal balance.
Investigations to be done are as below:
Urine pregnancy test if the period is delayed by more than five days.
Vaginal swab for microscopy and culture to confirm yeast or bacterial cause.
Basic blood work if infections recur (include sugar test since diabetes predisposes to recurrent yeast).
Differential diagnosis is:
Physiological discharge (but your discharge is foul-smelling and clumpy, so unlikely).
Trichomoniasis (but that usually gives frothy, greenish discharge).
Early pregnancy leukorrhea (but that is usually thin, milky, not itchy or smelly).
The treatment plan is as below:
Do a urine pregnancy test if the period is missed for more than five days.
For discharge: antifungal treatment (Fluconazole single‑dose tablet or Clotrimazole pessary course) is usually given. If bacterial vaginosis is also present, oral Metronidazole or vaginal gel may be needed.
Probiotics and vaginal hygiene advice.
Avoid using perfumed soaps, vaginal douches, and tight nylon underwear.
Partner treatment is usually not required unless the partner also has symptoms.
I suggest you consult a specialist, get yourself checked, and take the medications with their consent.
Preventive measures are as follows:
Do not use ECP repeatedly. For contraception, use regular methods, condoms, oral contraceptive pills, or consult for long‑term options like copper‑T or hormonal methods.
Maintain good hygiene; use cotton underwear and change frequently.
Avoid unnecessary vaginal washes and soaps.
If infections recur, check for diabetes.
Do a urine pregnancy test if the period is missed for more than five days from now, and share the report with me. If discharge persists even after treatment, you will need a vaginal swab culture. Please update me about your bleeding pattern after ECP because your period may come late, early, heavier, or lighter. If itching and smell worsen, you must see a gynecologist for examination.
I hope that this answers your query.
Kindly follow up if you have more doubts.
Thank you.
