Probable diagnosis

The probable diagnosis could be iron deficiency anemia, vitamin D deficiency, or vitamin B12 deficiency.

Treatment plan

I suggest you take the tablet Ferrous ascorbate and Folic acid (Orofer-XT) once daily after dinner for three months. Take a capsule of Cholecalciferol (Lumia 60K) once a week for 12 weeks. Also, take a tablet of multivitamin (Neurobion Forte) once daily after breakfast for three months.

Preventive measures

Take iron rich foods.

Regarding follow up

Kindly repeat the complete blood count (CBC), iron profile, vitamin B12, and 25-hydroxy vitamin D after three months and get back for follow-up.
