Patient's Query
Hi doctor,
I have been developing these lesions or wounds only on my right forehand for three to four months. I initially thought it might be mosquito bites; however, I have noticed these are increasing, and also became a blood boil recently, which cracked and started bleeding. There are a few big ones and a few smaller ones, and it does not cause any harm, pain, or redness.
Please advise.
Hi,
Welcome to icliniq.com.
I completely understand your concern.
Based on your history and the attached pictures (attachments hidden to protect patient identity), the lesions are papules with a dark scab in the center and dry skin around it, without obvious redness, swelling, or pain.
Because these spots have been present for several months, keep coming back in the same area, and one has started bleeding, they are unlikely to be simple mosquito bites.
It looks more like either a follicular-based infection, recurrent furunculosis, or hypertrophic prurigo.
I will give you some medicine to manage it. This medicine will work in both conditions.
Apply Fusidin H (Fusidic acid) cream on them day and night for seven to ten days.
Apply a thick emollient like Vaseline (petroleum jelly) on the whole arm to manage the scaling.
In case of itching, take a Cetirizine 10 mg tablet of cetirizine at bedtime.
Keep your arms covered, trim your nails, and avoid itching in these areas.
Use gentle soaps.
Get your glycated hemoglobin (HbA1c) checked, as early diabetes is associated with recurrent furunculosis.
Do this, and it will heal in 10 to 14 days.
Consult your specialist doctor, discuss with them, and take the medicines with their consent.
Additionally, I want to ask if you experience any itching on them, any history of weight loss, and what your occupation is, as localized disease is related to contact dermatitis. If the lesions still persist beyond 14 days, then you may need a biopsy by your local dermatologist to rule out other causes.
I hope my answer is clear to you, and if you need any details at any time, I am here and at your service.
Thank you.
