Hi doctor,

I have had these red spots or redness on the tip of my penis for over a year. When my foreskin pulls back, it also is sort of red. It does not itch or hurt, but sometimes it can get inflamed and then it does itch. Sometimes I get peeling on my foreskin too. The tip can be dry as well. I have tried an OTC antifungal, which did not do anything, and got it checked by a urologist and a dermatologist. Urologist told me to see a dermatologist, and the dermatologist told me it was a skin sensitivity. I think it is a bacterial infection of some kind.

How should I treat it? Please help.

I do understand your concern.

It appears to be Zoon's balanitis with a minor infection. I suggest you the following treatment.

  1. Apply Clotrimazole cream (candid cream) or 1 percent Miconazole gel twice daily for 10 days(on inflamed lesions of the glans).
  2. Wash the area with Episoft cleanser or No soap cleanser daily for 1 month.
  3. Tablet Zifi (Cefuroxime) 200 mg twice daily after food for five days(if irritation, or STI for bacterial infection, urine culture tests are positive).
  4. Tablet Teczine -M (Levocetirizine) once at night after food for seven days (if itching).
  5. Apply 1 percent Hydrocortisone gel at night for five days to reduce redness.
  6. Avoid harsh soaps or fragrance-based lotions/lubricants.
  7. Do not scratch or rub lesions.
  8. Avoid occlusive tight wear.
  9. Avoid chronic friction.

Check your sugar levels. Get swab tests and urine routine tests done.
