Do my symptoms indicate vasculitis?

Hello, Welcome to icliniq.com. I have read your query and can understand your concerns. The manifestations of vasculitis in general are weight loss of more than 10 % of body weight, aches and pains, fever, loss of appetite, and mononeuritis (a condition where damage or inflammation occurs to a single peripheral nerve). Investigations will reveal high ESR (erythrocyte sedimentation rate), positive CRP (C-reactive protein), high platelet count, and protein in urine, along with RBCs (red blood cells). The other manifestations depend on the type of vasculitis.

Is my burning sensation in my legs caused by a blood clot?

Hello, Welcome to icliniq.com. DVT (deep venous thrombosis) is a blood clot in the legs that does not cause a burning sensation but causes swelling of the affected leg, and you are already on blood thinners. I hope your INR (international normalized ratio) is within the therapeutic range unless you are taking new oral anticoagulants. I think you should do blood sugar and preferably HbA1c (glycated hemoglobin) and relax, as it cannot be DVT. I hope this helps.

I have fever and gurgling sensation near the liver area. Please advise.

Hello, Welcome to icliniq.com. I went through your post and investigations (attachment removed to protect patient identity). You have low hemoglobin and low T4 which meant you are having iron deficiency and your dose of Thyronorm is low although TSH is normal. Ideally, when TSH is normal T3 and T4 has to be normal but in your case, you need to be investigated for secondary hypothyroidism by doing a pitutary profile. For the time being, I suggest you increase the dose to 75 mcg Thyronorm.

Which medication should be taken to cure joint pain?

Hello, Welcome to iCliniq. I understand your concern. Sulfasalazine is a relatively mild disease-modifying antirheumatic drug and may not provide sufficient control in active autoimmune inflammatory disease. Methotrexate is more effective but usually takes around six weeks to start showing clinical improvement. A short course of oral steroid therapy, such as Methylprednisolone 4 milligrams once daily for ten days, can help control inflammation. The Methotrexate dose can be increased to 15 milligrams once weekly at bedtime, along with Folic Acid 10 milligrams the following day at bedtime.
