Is it rare to see sickle cell anemia and beta thalassemia together?

Hello, Welcome to icliniq.com. As per your description, your friend seems to be having sickle beta-thalassemia. It is common to see both these diseases together in communities where both diseases are prevalent. Actually, the severity of sickle cell disease comes down to the elevation of the HbF (fetal hemoglobin) component of hemoglobin, which is seen in thalassemia. Thus, high HbF will decrease the complications of sickle cell disease.

I would like to know about the following questions, Since when has your daughter experienced the bleeding manifestation? Do any of your family members have a similar history in their past? Does she take any chronic medication for pain? The most common bleeding disorder is a condition known as Von Willebrand's disease. I think she needs to be checked for it, and the other condition, which needs to be evaluated, is platelet dysfunction. Investigations suggested are, VWF: Ag (Von Willebrand factor antigen) assay.

Hi, Welcome to icliniq.com. From the information you provided, it seems that you have HbE beta thalassemia trait and your fiance has only Hb E trait. If so, then the chance of having a child with severe thalassemia is less. But, if you both have the same Hb E beta thalassemia trait, then there is a chance of having a beta-thalassemia major child. I would recommend you get a mutation analysis for both of you and consult a geneticist for more detailed counseling.

In which conditions does one have low serum iron?

Low MCV (mean corpuscular volume) means that your red blood cells (RBC) are small in size, which is more commonly seen in iron deficiency anemia. There are other conditions that show a similar laboratory profile such as thalassemia and other rare causes. As your serum iron level is normal, which probably excludes iron deficiency anemia, I would prefer you to do a blood test for thalassemia which is HPLC (High-Performance Liquid Chromatography).

What is the effective treatment for mild anemia in a hypertensive patient with hypothyroidism and osteoarthritis?

After going through all the clinical details and investigation reports (attachments removed to protect the patient's identity) of your mother, it looks like she has mild anemia with chronic diseases (hypothyroidism, osteoarthritis, and hypertension). Mild anemia is expected in people with such chronic diseases which are called anemia of chronic disease, but in that case, the peripheral smear usually shows normal-sized RBCs. Such large RBC (macrocytic) can be seen in many other conditions, common among them would be vitamin B12 and folic acid deficiency. Both reports are normal.
