Hi doctor,

I have been experiencing ongoing gastrointestinal symptoms, including upper abdominal pain, bloating, early satiety, and occasional nausea. Despite trying proton-pump inhibitors, my symptoms have persisted, which makes me concerned about possible underlying causes. I also notice that certain foods seem to worsen my condition. Please help.

I read your query and I understand your concern.

From your previous records and your complaints, I have the following diagnosis:

  1. SIBO (small intestinal bacterial overgrowth): The next step is to do a breath test for methane or hydrogen.
  2. Functional dyspepsia: Gut-brain axis problem, usually resistant to PPI (proton pump inhibitors).
  3. Gastroparesis: Delayed gastric emptying may be another cause of abdominal pain; consider doing a gastric emptying study.
  4. Autoimmune or inflammatory issues: It may require a battery of tests for antibodies.
  5. SMA (superior mesenteric artery) syndrome: It may require aortography.
  6. Median arcuate ligament syndrome: may require CT (computed tomography) or MRI (magnetic resonance imaging) angiography.

I suggest undergoing the following:

  1. UGI (upper gastrointestinal endoscopy) and colonoscopy.
  2. Trial of low FODMAP (fermentable oligosaccharides, disaccharides, monosaccharides, and polyols) diet.
