What causes persistent abdominal pain in a 19-year-old?
Patient's Query
Hello doctor,
I am a 19-year-old female. I am unmarried. I have persistent abdominal pain (upper and lower abdomen, mostly right side), bloating, stools with gas and water, and a feeling of nausea even after drinking water, sometimes back pain with abdominal pain, and recurrent throat pain every few days.
During abdominal pain, severe discomfort occurs to the extent that walking, sitting, or even changing position becomes very difficult.
-
Past reports: Ultrasound (abdomen): Clear.
-
CBC and LFT: Within normal range.
-
Colonoscopy: Normal.
Please help.
Thank you.
Hello,
Welcome to icliniq.com.
I can understand your concern.
Thanks for the details. I understand how exhausting and frustrating this must feel, especially since you have been dealing with these symptoms for so long without clear answers. You have been experiencing persistent abdominal pain, mostly on the right side, along with bloating, watery or gassy stools, and nausea.
In addition, you have had recurrent throat pain despite normal lab tests, ultrasound, and colonoscopy results. What makes this even more challenging is your long history of about seven years of constipation and ongoing stomach discomfort. At times, the pain becomes so severe that it limits your movement and daily activities, which can significantly affect your quality of life.
The reasons for your problems include:
-
Irritable bowel syndrome (IBS) is common with your symptoms and normal tests.
-
Functional gastrointestinal disorder (gut sensitivity without visible damage).
-
Less likely, but consider small intestinal bacterial overgrowth (SIBO) or mild food intolerances.
Given your symptoms, it would be best to consult a gastroenterologist for a more detailed evaluation. In the meantime, you can start maintaining a symptom diary to track your food intake, stress levels, and bowel habits, as this may help identify triggers.
Trying gentle dietary changes, such as a low FODMAP diet (which reduces fermentable sugars that can cause bloating and discomfort), may also provide relief. Additionally, managing stress and staying well-hydrated can support gut health. If your pain worsens or you notice any new symptoms, it is important to get re-evaluated promptly.
If you want, I can help you prepare questions for your doctor or suggest symptom-tracking tools.
I hope this helps.
Kindly follow up if you have more concerns.
Thank you.
