Hello doctor,
My daughter is experiencing abdominal pain and discomfort in her leg. Additionally, she is bleeding, which has persisted for several days, and she is not currently menstruating. Please tell me, what is causing this?

I got your concern about abdominal pain, abnormal bleeding for several days, and leg pain. Likely suggestive of anovulatory abnormal uterine bleeding (AUB, irregular bleeding from the uterus due to improper ovulation). On symptoms, you suggest it is likely to require an Immediate in-person gynecological evaluation to rule out serious conditions, including
Ectopic pregnancy.
Pelvic inflammatory disease.
Ovarian cyst.
Bleeding disorders.
Endometriosis.
Investigations to be done:
Urine or serum Beta-hCG (human chorionic gonadotropin) to check pregnancy.
CBC (complete blood count).
Coagulation profile.
Von Willebrand panel.
Cervical or vaginal swabs for gonorrhea and chlamydia.
Pelvic ultrasonography.
Treatment is given after assessing the hemodynamic status and the investigations given above. If the vital signs, such as blood pressure, pulse, and SpO2 (peripheral oxygen saturation) level, are unstable, immediate hospitalization is required. The following medicines can be given to her;
Oral iron supplementation.
NSAIDS (non-steroidal anti-inflammatory drugs).
Cyclic progesterone therapy.
High-dose hormonal therapy: OCP (oral contraceptive pills) or Medroxyprogesterone (a progestin).
