Hi doctor,

I am attaching the reports of my sister, who is complaining of pain in the abdomen. Locally she had an endoscopy and so on. I want your valuable suggestion for her further treatment.

Please suggest me things stepwise to understand them better.

1. What treatment does she require now?

2. Is endoscopic finding worrisome?

3. What endoscopic procedure does she require to get rid of her present problem?

4. Should the procedure be done in an emergency, or can we wait for a month, as we have festivals approaching?

5. However, if it is urgent we will move as per your suggestion.

6. What is her liver status, and is there anything required for the treatment of the liver?

I have reviewed your sister's case in detail. There are two issues,

1) Stomach related.

2) Liver-related.

First, I will talk about the stomach. I have reviewed the endoscopy report (attachment removed to protect the patient's identity), her esophagus and duodenum are fine, but there are gastritis or erosions and gastric polyps in the stomach. Gastritis or erosions are very common and can be medically treated with PPIs (proton pump inhibitors) and anti H.pylori treatment if RUT (rapid urease test) comes out to be positive, which you can follow if it is still less than 24 hours. Now coming to the main point, gastric polyps: this is something which is important and concerning in this endoscopy. They did not mention the size of the largest polyp or take a biopsy of any polyp to check for any metaplasia, dysplasia, or malignancy. They have suggested going for a polypectomy, and they will likely send a sample for histopathological examination as well. She should be on good PPIs (proton pump inhibitors) and things must be cleared related to her H.pylori status. We cannot leave gastric polyps as it is. So regarding time, one can wait for a few weeks, but this is something that should be done; you cannot leave it for months, especially when histopathology is not available yet.

Now coming to the second issue, her liver, 6.2 kpa, is not very bad considering her age of 60 years, but she should be evaluated for common liver problems like checking for hepatitis B and C (HBsAg and Anti-HCV) if not done yet. Sorry to ask, but is there any alcohol consumption or not? The last possibility can be fatty liver in her case. Considering her body weight and height, she will likely fall in the overweight category as per BMI (body mass index). So related to liver, nothing to worry about at the moment, just complete her workup. But yes, regarding her stomach, you should keep an eye on this thing. I hope this answer will help you out.
