I have gone through the history and images (the attachments are removed to protect the patient's identity) provided from your end. It appears as grade 2 acne vulgaris.
Acne is most often caused due to multiple factors including in case of genetic susceptibility, wherein there may be a similar history within your family that can be transmitted genetically due to hormonal fluctuations.
Especially during puberty and prepuberty, which can cause increased release of sebum within the skin, triggering bacteria causing acne due to stress and diet and climate factors.
Acne scars and hyperpigmentation is caused due to long-standing untreated acne, picking of acne, and in case of severe acne.
I suggest,
1. Ahaglow foaming face wash (Salicylic acid and Glycolic acid): To be used in the morning and at night. Apply a sufficient quantity, massage gently with circular motions for a minute, and wash off. It can be used for months.
2. Clinmiskin gel (Clindamycin and Nicotinamide) to be used in the morning on the acne spots after washing the area well. It helps with reducing pigmentation as well.
3. Clindac AP Gel (Adapalene and Clindamycin) to be used at night on the affected areas only in a very thin layer with a small quantity possible. It may be continued for 12 weeks or more.
4. Capsule Doxycycline 100 mg may be consumed after breakfast and dinner for seven days. Before consuming this medication, ensure there is no previous allergy to medications or antibiotics.
5. Tablet Pantop 40 (Pantoprazole) twice daily, 30 minutes before breakfast and dinner, to prevent acidity with medications for seven days.
6. Tablet Limcee (vitamin C) chewable tablet 500 mg once daily after food for one month.
7. Glycolic acid chemical peels help with reducing scars and pimples. This is a procedure in which a solution is applied for five to ten minutes on the face and washed off, and it is done every week for six sessions. This procedure is done at a dermatologist's clinic. Therefore, I request you consult a dermatologist to undergo the procedure.
8. The depressed scars noted may only be reduced with laser treatment which is done every three to four weeks for five to six sessions.
9. Avoid oily and fried food, sweets, red meat, milk, and dairy products. Make sure to include more fruits, vegetables, fiber, and at least 12 glasses of water daily.
Acne may be managed with medications and procedures when done in combination. They may appear on and off depending on the above factors and completely fade off once sebum production stabilizes.
