Hello doctor,

I have inflammatory acne, characterized by multiple red pimples beneath the skin on my face, particularly on my cheeks. They do not have white pus on top. My skin care includes Mentholatum face wash, Dermive moisturizer, Niaronic serum, LivGlow cream, and Spectramatt sunscreen. I started using these things recently. How can I treat them currently?

I understand how frustrating and emotionally draining it can be to have persistent, painful pimples under the skin, especially when they have been present for so long and do not come to a head. What you are experiencing is inflammatory acne, which commonly appears as deep red bumps on the cheeks and tends to last longer than surface pimples. Please be reassured that this is a very common condition, and it is absolutely manageable with the right approach and patience.

For now, keep your skincare simple and gentle. Continue washing your face twice daily with the Mentholatum face wash (Salicylic Acid, Isopropyl Methylphenol, and Lauric Acid), using only your fingertips and avoiding any scrubbing. This helps remove excess oil and impurities without irritating already inflamed skin.

Follow this with an oil-free moisturizer (containing Glycerin, ceramides, and Panthenol), as maintaining a healthy skin barrier is essential even if you have acne. You should temporarily pause the Niacinamide and Hyaluronic serum and Glutathione and Alpha Arbutin cream, as active ingredients can sometimes irritate inflamed acne and delay healing when the skin is already sensitive.

To actively treat the acne, apply Tretinoin and Clindamycin gel in the morning in a thin layer over the affected cheek areas. In the evening, apply a pea-sized amount of Azelaic acid cream under your moisturizer. These medications work by reducing inflammation, preventing new breakouts, and helping the existing deep pimples slowly settle.

It is important to avoid squeezing, picking, or rubbing these pimples, as this can worsen inflammation and lead to marks. Also, avoid facials, scrubs, or home remedies during this phase. Improvement with this type of acne is gradual, so do not be discouraged if changes feel slow at first.

Pain and redness start reducing within two to three weeks. Once the active acne is controlled, you can start with serum for acne marks, and pigmentation can be started safely and effectively, as using too many actives at the same time can irritate the inflammatory acne, so please add the serum and whitening cream once your acne settles fully to achieve maximum results.

Continue this regimen for at least one month and follow up if needed.
