Hello doctor,
I had a very painful bowel movement and saw swelling on the left side of the anus that caused agony. It has improved but then flared again, therefore I am going to see a colorectal surgeon because sitting is unpleasant. Please look at the photos and establish if this is an external hemorrhoid.

I reviewed your history and pictures (the attachments removed to protect the patient's identity). It appears to be an anal fissure with some abscess formation. It usually occurs due to the passage of hard stools. But there is nothing to worry about.
You might probably have constipation or an anal fissure with a tag or abscess. So the treatment plan for you may include:
Ispaghula Husk 2 TSF at night in one glass of water.
GTN (Glyceryl trinitrate) plus ointment and 2 % Diltaizem ointment. Apply both to this area three times daily.
Syrup Cremaffen twice a day.

I am concerned this is anal cancer. I know you do not know for certain but what is your opinion based on this picture and how it occurred. I have never had any bleeding and bowel movement have been regular.

You do not need to worry about it. There are no alarming indicators of cancer. Constipation is frequently the cause. Please begin treatment as I recommended, and it will resolve in a few days; you can then see your colorectal surgeon.
