Patient's Query

Hello doctor,

I recently had a bout of diarrhea immediately after returning from a trip, during which I exclusively consumed food and beverages at a luxurious five-star resort. The resort assured that all drinks and ice were prepared using purified bottled water. My return journey involved 21 hours of travel. As of today, which is a week after my arrival, I have been experiencing persistent itching and stinging in my anal area, and it appears to be worsening. While my stool has been mostly regular, it has been a bit loose or soft at times, although not severely so. There are no visible signs of irritation, no presence of blood in my stool, and the discomfort is more pronounced after bowel movements, especially at night. My only remedy has been applying Aquaphor in an attempt to help with the itching, and I have not taken any other medication for these symptoms. Please help.

Thank you.

Hello,

Welcome to icliniq.com.

I understand your concerns and would like to provide some guidance.

Based on the information you have provided, your symptoms are likely related to hypersensitivity, which is a common occurrence following an episode of infectious diarrhea. A deficiency in vitamin B12 can also produce similar symptoms. In some cases, engaging in anal intercourse can also lead to comparable discomfort.

To assist you further, kindly provide the following information:

  1. Have you undergone any relevant tests, such as those to assess your vitamin B12 levels?
  2. I apologize for the personal nature of this question, but it is important for a comprehensive evaluation. Are you sexually active, including engaging in anal intercourse?

Kindly respond with the answers to these questions so that I can offer more precise guidance.

Thank you.

Patient's Query

Hi doctor,

Thank you for replying,

I have not engaged in anal intercourse recently. My sexual activity is limited to my boyfriend, and we have not had sexual intercourse in approximately 2.5 weeks. I have not undergone testing for low vitamin B12 levels.

Thank you.

Hi,

Welcome back to icliniq.com.

I suggest you get a vitamin B12 level test done, and if it shows a deficiency, you may need to take supplements. In the meantime, I would suggest you apply Anovate (Beclomethasone, Phenylephrine, and Lidocaine).cream both before and after passing stool. Have you noticed if the skin around your anus has become dry to some extent? Also, are you currently taking any multivitamin tablets or supplements?

Thank you.
