Hello doctor,

I have been having an itchy anus for like three months now. I have tried deworming tablets, over-the-counter cream, and antifungal tablets, but all in vain, and this happens more especially after visiting the toilet, and the irritation starts. What can I use?

This kind of itching, known as pruritus ani, can be caused by several factors, including fungal or bacterial infections, pinworms, hemorrhoids, anal fissures, irritation from soaps or wipes, excessive moisture, or even certain foods (like spicy foods, caffeine, or dairy). Since it worsens after using the toilet, it may also be related to irritation from wiping, incomplete cleaning, or stool leakage.

For now, keep the area clean and dry. Wash gently with warm water (avoid soap or wipes that contain fragrance or alcohol), and pat dry instead of rubbing. You can apply a mild Zinc oxide barrier cream (like Calamine Zinc cream) to protect the skin and reduce irritation. Avoid scratching, as it worsens inflammation. Wear loose, cotton underwear and change them daily.

If you have not yet, it would be good to have a doctor examine the area to rule out hemorrhoids, infection, eczema, or other causes. Sometimes, a short course of a mild steroid-antifungal combination cream (like Hydrocortisone with Clotrimazole) is prescribed to calm inflammation and treat any underlying infection.

If symptoms persist, a stool test for ova and parasites, and a swab test for fungal or bacterial infection around the anus might help find the exact cause. Also, ensure you have treated all family members if worms were suspected. With proper diagnosis and gentle care, this usually settles completely.
