Why does one have anal pain after bowel movements?

Hi doctor,

I had a very large bowel movement that got stuck, and I had to push it hard to get it out. Afterwards, I experienced a pain in my anus. There is a large swelling. It has not gone down despite many treatments. I am going to a surgeon, but I wanted to get your opinion on whether this is an external pile. It causes pain while sitting. Please help.

It looks like an external pile (small lumps just outside the anus that can cause bleeding and itching), and the pain could be due to a combined fissure (a small tear in the thin, moist tissue that lines the anus). External piles usually do not cause much pain.

The treatment is to:

1. Relieve constipation (a condition where a person is passing fewer than three stools a week or has difficult bowel movements) with laxatives.

2. Local anesthetic cream for local application.

3. Use painkillers for seven days.

4. Do regular exercise, meditation, and yoga.

I think you can wait before opting for surgery at present.
