How can a 21-year-old manage male pattern baldness?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hello doctor,

I am a 21-year-old male. I took Isotretinoin (retinoid, vitamin A derivative) for just two weeks. Since then, I have experienced severe hair loss in all areas, including the front, crown, sides, back, and even the eyebrows. I discontinued Isotretinoin and have since started taking finasteride 1 mg (5-alpha-reductase inhibitor, oral) and using Minoxidil 5% (vasodilator, topical) for the past two months.

However, the hair shedding has not stopped; in fact, it appears to be worsening. My blood work has come back normal. I also take Biotin and a general vitamin supplement. I do have a family history of androgenetic alopecia (male pattern baldness), so I am concerned that Isotretinoin may have triggered or accelerated the process. If anyone here has gone through something similar, particularly hair loss associated with Isotretinoin, I would appreciate hearing about your experience.

  1. Will the hair eventually grow back?
  2. How long will it take?

Any advice, shared experiences, or support would mean a great deal to me right now.

Kindly help.

Hello,

Welcome to icliniq.com.

I have read your query and can understand your concern.

Thank you for sharing your experience. Hair loss after a short course of Isotretinoin (Retinoid, vitamin A derivative) can be very distressing, especially when it is severe and persistent.

Here is some important information and advice regarding your situation:

Isotretinoin and hair loss. Hair loss, typically telogen effluvium, is a known but uncommon side effect of Isotretinoin (Retinoid, vitamin A derivative). It usually begins a few weeks to a few months after starting or stopping the medication. This type of hair loss is often temporary and caused by a disruption in the hair growth cycle. For most individuals, hair regrowth occurs within three to six months after the trigger has been removed.

Current treatment:

Finasteride and Minoxidil. Finasteride 1 mg (5-alpha-reductase inhibitor, oral) and Minoxidil 5% (vasodilator, topical) are standard treatments for androgenetic alopecia (male pattern baldness). Given your family history, it is possible that Isotretinoin triggered or accelerated underlying androgenetic alopecia. These treatments require consistency and patience. Visible improvement typically takes four to six months, sometimes longer. Continue both medications as prescribed. Discontinuation may lead to increased shedding.

When to seek further evaluation. Since the hair loss has persisted and worsened for over two months after stopping Isotretinoin, it is important to consult a dermatologist or hair specialist for a detailed assessment. Consider a scalp examination and possibly a scalp biopsy if the diagnosis remains uncertain. Rule out additional causes such as thyroid dysfunction, nutritional deficiencies, or autoimmune conditions.

I suggest you do the following:

Maintain a balanced diet rich in protein, iron, zinc, and vitamins essential for hair health. Avoid tight hairstyles or harsh chemical treatments that may contribute to traction alopecia. Practice stress management techniques, as psychological stress can aggravate hair shedding. Be patient, as hair regrowth is a gradual process and may take several months.

Hair loss associated with Isotretinoin is usually reversible, although regrowth can take several months. Continue Finasteride and Minoxidil under medical supervision. Follow up with a dermatologist for a thorough evaluation to ensure all possible causes are addressed. Monitor for any new or worsening symptoms and adjust treatment accordingly.

Please feel free to ask further questions or seek professional guidance if needed.

I hope this helps.
