How do I manage recurrent hives and swelling episodes?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hello doctor,

I keep getting itchy, raised red welts all over my body that appear suddenly and disappear within hours or days. These hives are extremely itchy, and sometimes my lips or eyelids swell along with them. I cannot figure out what’s triggering these reactions, as they seem to happen randomly throughout the week. The itching is so intense that I scratch until my skin bleeds, and antihistamines only provide temporary relief. What could be causing these recurring hives and swelling episodes?

Kindly advise.

Hello,

Welcome to icliniq.com.

I understand your concern.

The symptoms you describe, such as recurrent itchy red welts that appear and fade within hours or days, sometimes accompanied by swelling of the lips or eyelids, are characteristic of urticaria. The associated swelling is called angioedema.

In many cases, hives can be triggered by allergic reactions to foods, medications, insect bites, or environmental factors. They can also occur due to non-allergic triggers such as stress, infections, temperature changes, or pressure on the skin. When hives recur frequently without a clear trigger, the condition is often referred to as chronic spontaneous urticaria (CSU), where the immune system causes the release of histamine in the skin without a definite external cause.

While antihistamines (such as Cetirizine, Loratadine, or Fexofenadine) are usually the first-line treatment, some people may require higher or regular doses. In resistant cases, other therapies such as leukotriene inhibitors (like Montelukast), corticosteroids for short-term use, or biologic treatments like Omalizumab may be considered under specialist care.

Because your symptoms include angioedema and significant discomfort, it is important to consult an allergist or dermatologist for proper evaluation, which may include reviewing your medical history, identifying possible triggers, and sometimes performing blood tests or allergy testing.

Until then, keeping a symptom diary to track potential food, medication, or environmental associations, and avoiding known irritants, may help in identifying patterns and reducing flare-ups.

I hope you are satisfied with my answer. For further queries, you can consult me at iCliniq.

Thank you.
