Hello doctor,

I am a 57-year-old diabetic and hypertensive patient. I am currently on medications. I have an ankle fracture involving a fracture of the fibula. I had MI last year and was diagnosed with heart blockages. I am on Aspirin and having swelling at the trauma site. Can this fracture heal without operation?

I have seen the X-rays and understand the pattern of the fracture (attachment removed to protect the patient's identity). Ideally, surgery should be done as apart from fracture, there is mild subluxation of the joint. Now the question is fitness for the procedure and when it should be done. Fitness has to be decided in conjunction with an anesthetist and physician so that they can examine it in detail and let the surgeon know what is the correct approach. Also, blood sugars will have to be managed with insulin during hospitalization followed by appropriate medications as determined by the diabetologist at the time of discharge. As of now strict limb elevation, intensive ice packs, and appropriate medications need to be given to decrease swelling, and pain and make skin conditions suitable for a surgical procedure. Kindly send me clinical pictures of the involved ankle so that I can understand what skin looks like.
