Is bariatric surgery safe for those on anticoagulants?

Hello doctor,

I have been diagnosed with a rare condition known as antiphospholipid antibody syndrome, which causes my blood to clot abnormally even in the absence of cuts or injuries. As a result, I am prescribed anti-coagulant medication. Additionally, I struggle with being overweight due to reduced lung capacity. Given my medical history, including pulmonary emboli and surgeries, along with complications such as high blood pressure leading to kidney issues like lupus nephritis, I am currently managing my condition with a combination of medications including Warfarin, Hydroxychloroquine, Mycophenolate, and Lisinopril. In light of my complex medical situation, I am considering options like invasive gastric surgery or a gastric balloon to address my weight concerns. However, I am seeking advice on whether these procedures would be suitable considering my health challenges.

I understand your concern.

In bariatric surgery, there is a chance of thromboembolic events, especially if a previous event is present. Given your history of pulmonary embolism and current use of anticoagulants, you are at high risk. Therefore, in my opinion, bariatric surgery is not advisable in your case. However, if surgery appears necessary, I recommend consulting a nearby hematologist with your coagulation reports for further discussion.
