What are the ways to manage anxiety in a 35-year-old?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hello doctor,

I am 35 years old this month and have suffered from severe anxiety since age 3. Life is a struggle daily, I exist, and I get through the days and work. But it is so hard. My anxiety caused me to settle for a not-very-good husband (I had no friends, and it felt like a miracle to have someone of my age to spend time with).

But he is not supportive, and I suspect he is on the autism spectrum, as now three of our four children are too. Every day is a battle. They are six, four, and twin 2.5-year-olds. But lately, I have been through a lot, including major gastric surgery for peritonitis and developed sepsis after a ruptured appendix, and ended up in the ICU.

They cut me right down the middle as they thought it was a perforated bowel and checked every inch. My autistic 4-year-old is stuck in the mainstream, and it is a daily effort and stress to get her into a proper environment. She lacks risk perception and is nonverbal.

The pastoral care at her school reported me to social services when I explained that I needed hook and eye closures on upstairs doors so she could not climb furniture or get into the toilet during the day. This led me to be physically ill for five days and prevented me from sleeping due to concern.

They could not understand my reasons. They were not for locking children in, but for keeping them out during the day. I had a small vehicle accident, but it was caused by my being in tears when driving over the social services and feeling betrayed by pastoral care. I am basically on constant alert. I can not sleep, relax, or focus on anything I need to do.

I used Diazepam for a short period of years ago to treat anxiety. My doctor did not prescribe it to me. They have prescribed Mirtazapine. This takes six weeks to get into the system, which is not going to help me with my current problem. Is there a method to obtain a private prescription?

Kindly help.

Hello,

Welcome to icliniq.com.

I read your query and can understand your concern.

I am sorry to hear about the immense stress and anxiety you are experiencing. It sounds incredibly challenging, especially with your recent health issues and the demands of caring for your children. Here are some steps you might consider:

  1. While Mirtazapine can take several weeks to become effective, other short-term options might help. Discuss with your GP (general practitioner) about the possibility of a short-term prescription for a benzodiazepine like Diazepam, especially given your previous positive response. If your GP is unwilling, seeking a second opinion or a private consultation might be an option.
  2. Cognitive behavioral therapy (CBT) can be very effective for managing anxiety.
  3. Reach out to support groups for parents of children with autism spectrum disorders. Connecting with others who understand your situation can provide emotional support and practical advice.
  4. Try to prioritize self-care, even in small ways. This could include mindfulness exercises, relaxation techniques, or finding moments of rest whenever possible.
  5. Consider consulting a psychiatrist who can provide a comprehensive evaluation and suggest a tailored treatment plan, including medication adjustments if necessary.

Kindly book a consultation with me if you would like more detailed information on any of these points, or if you have other questions.

I hope this helps.

Take care.

Regards.
