I have white circles on my vaginal lip interior. How to manage?

Hi doctor,

I just realized I have three weird white circles on the interior of my vaginal lip. I started noticing a bit of discomfort a couple of days ago because it would rub against my underwear. Should I be worried?

I do currently have a long-term partner whom I have been with for three years. We had protected sex a week ago, and before that, we had not seen each other for a few months. I have never had these before that I am aware of, and I do not know what they are.

Please help.

I understand your concern.

I reviewed the description and the image (attachments hidden to protect patient identity). Thanks for attaching the picture too. It makes the diagnosis even clearer.

Before that, I would like to know whether you have any oral ulcers as well. Any complaint of fever, cold cough, recent change of soap, use of anything in that area, or change in type of condom.

So, based on your history and image, this appears to be acute non-sexually transmitted vulvar aphthous ulcers (Lipschütz ulcers). This is typically seen in adolescents and young women (ages 15 to 25). This is usually with mild discomfort and is not painful. It is not accompanied by fever or itching. It has no vesicles and appears as shallow, whitish, or yellow ulcers.

They can occur with or without sexual activity; they often appear days or weeks after stress, minor illness, or even spontaneously. So do not worry, but be vigilant for at least two weeks. And for the management part, I would suggest local care:

  • Vulvar rest means avoiding intercourse for one to two weeks.

  • No soaps, antiseptics, or washes.

  • Use loose cotton underwear.

  • Use plain emollient (petroleum jelly) to be applied over lesions.

  • Apply Lignocaine gel only if pain increases.

The expected course of this lesion generally starts healing in seven to 10 days, and complete resolution is seen in two to three weeks, with no scarring. Recurrence is uncommon. If you notice it is not healing beyond three to four weeks and the pain is increasing, consult the doctor again.

Recurrence means once these lesions heal, they again come back. Induration or bleeding means becoming large, hard, and bleeding.
