Hello doctor,
I have been suffering from recurrent wheezing and excessive phlegm for the past three years. I also have excessive mucus in my throat, and my voice becomes low or hoarse while speaking.
Currently, I am taking Montair LC twice daily and Methylprednisolone 8 mg as needed, but I am not getting much relief.
What could be causing these persistent symptoms, and why are the medications not helping adequately?

One possible cause is bronchial asthma or chronic allergic airway disease. Montair LC, which contains Montelukast and Levocetirizine, may help with allergic symptoms, but many patients with persistent asthma symptoms require inhaled corticosteroid therapy for proper long-term airway control.
Other possible contributing conditions include chronic sinusitis with postnasal drip, allergic bronchitis, or laryngopharyngeal reflux, where acid reflux irritates the throat and airways even without typical heartburn symptoms. These conditions can cause chronic phlegm, throat clearing, cough, and voice changes.
Repeated use of oral steroids such as Methylprednisolone without proper long-term control is generally not recommended because it may lead to side effects and does not adequately manage chronic airway inflammation.
Further evaluation would be important. Investigations such as a chest X-ray, spirometry with bronchodilator testing, eosinophil count, and total IgE (immunoglobulin E) levels may help identify the underlying cause. An ENT evaluation may also be useful to assess throat irritation, sinus disease, or postnasal drip.
If asthma is confirmed, treatment often includes a daily inhaled corticosteroid with or without a long-acting bronchodilator rather than relying only on tablets or intermittent oral steroids.
