How can eczema flare-ups be managed effectively?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hello doctor,

I am a 26-year-old female with a history of atopic dermatitis since childhood. Recently, I’ve noticed that the itching and redness on my neck and hands have worsened, particularly in hot weather. At times, the skin also becomes thickened and very sensitive.

Could environmental factors like heat or sweating contribute to these flare-ups? I also want to know if topical steroid creams are safe for repeated use during severe itching episodes.

Please help.

Thank you for your time and help.

Hello,

Welcome back to icliniq.com

The symptoms you described, such as worsening itching and redness on your neck and hands during hot weather, along with times when your skin becomes thickened and sensitive, match what we see in adult atopic dermatitis. This long-term condition can come and go, and it is often affected by things like your environment and daily habits.

Things like heat, humidity, and sweating are known to trigger eczema. With atopic dermatitis, the skin’s barrier is weaker, so it reacts more easily to things that can irritate it. Heat and sweat can make symptoms worse in a few ways:

  • Sweat has salts in it, which can make already irritated skin feel worse.

  • When your body gets warmer, it can make the itching feel stronger.

  • Moisture that gets trapped in skin folds, like on your neck, can make inflammation worse.

  • When you sweat, your clothes can rub against your skin and cause more irritation.

Because of this, many individuals notice flare-ups during hot weather or in humid climates.

If you scratch a lot over a lengthy period of time, your skin can get lichenified. The skin becomes thicker, rougher, more sensitive and deeper lines appear. This is because the persistent inflammation and the itch-scratch cycle keeps the symptoms going.

Topical corticosteroids are among the most effective treatments for eczema flare-ups. When used correctly under medical guidance, they are generally safe and effective. A few key considerations include:

  • Using the appropriate potency depending on the body area (for example, the neck and hands may require different strengths)

  • Applying them for short, controlled periods during active flares rather than continuously

  • Avoid prolonged use of strong steroids on delicate areas like the neck or face because this may cause skin thinning.

To reduce frequent steroid use, dermatologists may recommend steroid-sparing options like Tacrolimus or Pimecrolimus. These help control inflammation and are especially useful for sensitive areas like the neck.

Helpful measures to reduce flare-ups

  • Use a thick, fragrance-free moisturizer a few times each day to keep your skin barrier healthy.

  • Take lukewarm showers instead of hot baths.

  • Wear loose, breathable cotton clothing.

  • Rinse off sweat and reapply moisturizer afterward

  • Avoid fragranced products and harsh soaps

When further evaluation may be needed

If eczema becomes persistent, significantly thickened, or difficult to control with standard topical treatments, additional options such as phototherapy or advanced therapies may be considered by a dermatologist.

Your symptoms worsen with weather changes and affect areas like your neck and hands. It can help to review your skincare routine, possible triggers, and past treatments. This will make it easier to find a long-term plan that works for you.

If you feel okay with it, sharing the information below can help me give you advice that fits your needs.

  • Photos of the affected areas

  • A list of any medications or moisturizers you are currently using

  • Please tell me if itching is making it hard for you to sleep

  • Let me know about any triggers you are aware of, such as certain cosmetics, metals, or detergents

This information will help me suggest a plan that is more tailored and effective for managing your eczema.

I hope this addresses your concerns.

Please feel free to follow up if you need any additional clarification.

Thank you.
