Hi doctor,

I have been dealing with a condition that has been bothering me for about three years now. I have noticed a change in my body odor. I smell like pop and sewage. Additionally, when I brush my teeth in the morning, I feel itching around my upper body.

There is also a painless swelling in my upper mouth, and my skin produces much more oil than it used to. Furthermore, I have three sores on my shoulder that have refused to heal for over eight months.

I am deeply concerned about your worries regarding urticaria. If you have experienced red or skin-colored bumps that appeared and disappeared quickly, it is unlikely to be simple bug bites.

This skin rash could be hives, and the itching associated with hives may range from mild to severe. Hives, also known as urticaria, affect about 20 percent of people at some point in their lives.

Factors such as scratching, alcoholic beverages, exercise, and emotional stress may worsen the itching.

Hives, also known as urticaria, are characterized by raised itchy bumps that can be either red or skin-colored. A common feature is "blanching," where the center of a red hive turns white when pressed.

Various triggers can lead to hives, including certain foods, particularly peanuts, eggs, nuts, and shellfish, as well as medications like antibiotics, Aspirin (Acetylsalicylic Acid), and Ibuprofen.

Other triggers include insect stings or bites, physical stimuli such as pressure, cold, heat, exercise, or sun exposure, latex, blood transfusions, bacterial infections (including urinary tract infections and strep throat), viral infections (such as the common cold, infectious mononucleosis, and hepatitis), pet dander, pollen, and some plants.

To investigate hives, a complete blood count (CBC) and serum IgE levels may be conducted. For management, it is advisable to consult a dermatologist.

Treatment may include:

  1. Take Kestine (Ebastine) 10 mg, one tablet three times daily for one month.

  2. Apply Calamine lotion, one application three times daily for one month.

  3. Using Dermavate (Clobetasol Propionate) cream, also one application three times daily for one month.

For about three years now, I have been experiencing a smell similar to poop and drainage. People cover their noses when I enter a space, whether it is open or closed, and this situation seems to be getting worse over time.

Additionally, I have lost my sense of smell and taste. The attached picture shows a sore that has not completely healed for about eight months now.

I read your query and understand your concern.

Based on your symptoms, the possible causes of your symptoms are:

1. Chronic nasal or sinus disease - Chronic sinusitis or atrophic rhinitis (ozena) can cause a foul body odor from infected nasal secretions. Loss of smell or taste, facial heaviness, nasal blockage, and odor are consistent with this. A painless swelling in the upper mouth (palate) could be due to sinus disease spreading into the oral cavity.

2. Chronic infection or immune disorder - Non-healing sores on the skin (eight months old) suggest an immunocompromised disease. This could be due to diabetes, chronic bacterial or fungal infection, or an immune deficiency. Chronic infections in the sinuses and skin can explain the odor and sores together.

3. Cancer - A painless swelling in the mouth and non-healing ulcers raise concern for malignancy (e.g., squamous cell carcinoma). Cancer in the nasal cavity, palate, or sinuses can cause foul odor, obstruction, loss of smell, and chronic sores.

4. Systemic or metabolic conditions, such as the following:

  • Diabetes mellitus: Causes recurrent skin infections, poor healing, oily skin, and sometimes body odor changes.

  • Liver or kidney disease: Can cause unusual odors due to the buildup of toxins.

  • Hormonal changes (thyroid, especially hyperthyroidism, androgen excess): Can increase sweating and oily skin.

These strongly suggest that more than just sinus issues are going on. Cancer and chronic infection must be ruled out urgently.

I suggest you undergo the following baseline investigations:

  1. Complete blood count.

  2. Liver function test.

  3. Renal function test.

  4. Serum electrolytes.

  5. Viral markers.

I suggest you take these medications:

  1. Tablet Augmentin (Amoxicillin and Clavulanic Acid) 625 mg twice daily for seven days.

  2. Ticovate (Fluticasone Propionate) nasal spray, two drops twice daily for three weeks.

  3. Nilstat (Nystatin) oral wash thrice daily.

I understand your concern.

I have seen all the test reports and the culture and sensitivity test (attachments removed to protect the patient’s identity). I have also seen the drugs that you are resistant to and sensitive to.

How is your condition now after taking the medications for almost one month? I will recommend you to take the following medication: the first four medications, followed by two weeks of oral medications.

  1. Injection of Amoxicillin 1g intravenously twice a day for three days.

  2. Oral medications: Tablet Amoxicillin 1g twice a day for two weeks. Take tablet Levofloxacin 500mg twice a day for 14 days.

Hello doctor,

Thank you for the reply.

I forgot to tell you that I took Streptomycin injection for five days, also Augmentin tabs before the injection, and I am currently using Dermavate cream. I could not access the others yet.

The sores on my shoulders are drying up. The area under my feet feels sticky on the floor tiles, especially after bathing. The smell is still present, and my sense of smell and taste has yet to return.
