Are IUDs a safe long-term birth control option?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hello doctor,

I am looking for long-term birth control that does not require daily pills since I forget them and worry about pregnancy. I want to understand different IUD types, including hormonal (Mirena, Kyleena, Skyla) and non-hormonal (copper or Paragard), and how long each lasts.

I am concerned about side effects, especially whether they will worsen my already heavy, painful periods, cause weight gain, mood swings, acne, or affect my libido.

I have heard conflicting information about IUDs being safe for someone who has never been pregnant.

  1. Does IUD insertion hurt?

  2. Is anesthesia available?

  3. How quickly can I resume activities?

  4. Can it move or fall out?

  5. What are expulsion signs?

  6. Can my partner feel it during sex?

  7. How soon am I protected?

  8. Can IUDs cause infertility?

  9. When can I get pregnant after removal, and what is the failure rate?

Please suggest.

Hello,

Welcome to icliniq.com.

Understandably, you want a long-term and low-maintenance birth control method, especially when remembering daily pills is stressful. Intrauterine devices are very reliable options, and they are safe for women who have never been pregnant.

Hormonal IUDs (intrauterine devices) such as Mirena, Kyleena, and Skyla release a small amount of Levonorgestrel inside the uterus. Mirena lasts about eight years, Kyleena lasts about five years, and Skyla lasts about three years.

These hormonal IUDs usually make periods much lighter and less painful, and many women have shorter periods or even stop bleeding altogether with time.

Hormonal IUDs generally do not cause weight gain, mood swings, acne, or changes in libido because the hormone stays mostly inside the uterus and very little enters the bloodstream.

The copper IUD, also called Paragard, is non-hormonal and lasts about ten to twelve years. It is very effective, but it can make periods heavier and more painful, especially in the first few months.

So it may not be the best option for someone who already has heavy or painful cycles. Insertion can be uncomfortable for some women, but many tolerate it well.

You can ask for pain medicine before the procedure, and some clinics also offer a numbing injection to help reduce cramping during insertion.

The discomfort usually lasts only a few minutes. Most women can return to normal activities the same day, although mild cramping and spotting for a short time are common.

The IUD rarely moves or falls out, but if it does, you might notice stronger cramps, unusual bleeding, or the strings feeling longer or missing. Your partner normally cannot feel the IUD during sex, although the strings may be felt slightly at first. These soften with time.

Protection from pregnancy starts immediately for the copper IUD and within seven days for hormonal IUDs. IUDs do not cause infertility, and once the device is removed, you can become pregnant right away if you wish.

The failure rate is extremely low, less than one percent, making IUDs some of the most reliable birth control methods available. If you want, I can help you compare these options based on your specific cycle patterns and personal preferences.

I hope this helps.

Regards.

Important Considerations

Medications should be taken after consulting a physician. The dosages will be prescribed by the physician. Do not alter, start, or stop medications without consulting a healthcare professional. While IUDs are highly effective, discuss all your health concerns and preferences with your doctor to choose the best option for you.

This information is based on general medical guidance. It is not a substitute for professional medical advice; consult a qualified clinician.

Always consult a doctor before taking medication; self-medication carries serious health risks. Take exact prescribed doses, and never start, change, or stop treatment without medical supervision.
