Is HRT safe in premenopausal women with metastatic cancer?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hello doctor,

My wife is 42 and was diagnosed with bladder cancer, which has now spread to her liver. She is currently undergoing chemotherapy, but her periods have completely stopped, and she is experiencing severe hot flashes and vaginal dryness. Her oncologist mentioned that the treatment might induce early menopause, but we were not prepared for how significantly it would affect our intimacy. Should she ask about hormone replacement therapy, or could it interfere with her cancer treatment? I am also concerned about bone density loss, as she has already lost 15 pounds and feels very weak. Is there anything safe she can take to help manage these menopausal symptoms?

Please help.

Thank you.

Hello,

Welcome to icliniq.com.

I understand your concern.

Hormone replacement therapy (HRT) and chemotherapy for bladder cancer are generally not directly linked in a way that HRT would interfere with chemotherapy. The decision to use HRT with bladder cancer chemotherapy should be made on an individual basis, considering the specific type of HRT, the patient's overall health, and the potential benefits.

Both menopause and certain chemotherapy treatments for bladder cancer can significantly contribute to decreased bone density and increased risk of osteoporosis, leading to weaker bones and increased fracture risk. Chemotherapy can directly damage bone cells and alter hormone levels, while menopause naturally leads to a decrease in estrogen, both crucial for bone health. For safe treatment of weak bones in menopause and chemotherapy, consider medications like Bisphosphonates or Denosumab.

Lifestyle changes, such as weight-bearing exercise, a balanced diet rich in calcium and vitamin D, and avoiding smoking and excessive alcohol consumption, can also be beneficial.

I hope this helps.

Kindly follow up if you have more concerns.

Thank you.
