Hello doctor,

I am 67 and have bladder cancer, and my doctor says bladder removal might be the best option now. But I am confused and worried. Does quality of life really improve after bladder removal at 67?

I think my quality of life would become worse, and I am terrified of ending up with a urostomy bag and losing my independence.

If I believe bladder removal offers you the best chance of curing or controlling your bladder cancer, I recommend it despite the life-changing nature of the surgery. Many patients worry that their quality of life will be worse, but in reality, most adapt remarkably well and regain their independence.

Recovery usually takes six to 12 weeks, with continued improvement in strength and stamina over several months. Once you have recovered and become comfortable with your urinary diversion, you can usually travel, exercise, play sports, drive, and enjoy a very active lifestyle. Having a urostomy (a surgical procedure for urine passage) should not prevent you from doing the things you enjoy.

The first few months are the most challenging because there is a learning curve with stoma care, but it becomes routine with time. Most of my patients tell me that once they recover, they are relieved to have the cancer treated and can get back to living their lives.

My advice is to make your decision based on the best chance of curing the cancer, not on fear of the urostomy.

At 67, if you are otherwise reasonably fit, you have every reason to expect a good recovery and an excellent quality of life after surgery.
