How can my husband, 58, manage the chemo side effects?

Hello doctor,

My 58-year-old husband was diagnosed with metastatic bladder cancer that has spread to his liver and bones, and we are devastated by the prognosis.

The oncologist started him on Cisplatin-based chemotherapy, but he is experiencing severe side effects, including hearing loss, kidney problems, and neuropathy in his hands and feet. He can barely walk without assistance, and the bone pain keeps him awake most nights despite strong pain medications.

While the chemotherapy seems to be shrinking some tumors, it is also causing new problems that are making his quality of life unbearable. We have heard about immunotherapy drugs such as Pembrolizumab, but insurance is denying coverage, stating that he must first fail all first-line treatments. He has lost 25 pounds and has no appetite, which is making him weaker every day.

The doctors speak in terms of months of survival rather than years, which is heartbreaking for our entire family. Are there any clinical trials or experimental treatments he might qualify for? We are willing to travel anywhere or try anything that might give us more time together. This disease is progressing so quickly, and we feel helpless.

I am truly sorry you and your husband are facing this. It is an incredibly difficult journey, and the side effects you describe from Cisplatin are unfortunately common when it is given at advanced stages.

For metastatic bladder cancer that has spread to the liver and bones, immunotherapy with drugs like Pembrolizumab or Nivolumab can be very effective in some patients and is often used after, or even instead of, chemotherapy when toxicity is too severe.

If insurance is resisting coverage, your oncologist can sometimes appeal on the basis of intolerable side effects rather than treatment “failure,” so it is worth pushing for that.

Beyond immunotherapy, there are targeted therapies such as Erdafitinib for tumors with FGFR 2/3 (fibroblast growth factor receptor) alterations or antibody–drug conjugates like Enfortumab vedotin and Sacituzumab govitecan, which have shown benefit even after chemotherapy.

Given his rapid decline and weight loss, a referral to a comprehensive cancer center could open the door to clinical trials, where combinations of immunotherapy, targeted drugs, or novel agents may be available. Many of these centers also have programs to help families with travel support.

At the same time, integrating palliative care alongside oncology can help manage pain, appetite loss, and sleep so that his days are more comfortable while pursuing treatment.

While the prognosis is hard to hear, there is still hope through trials and newer therapies, and seeking a second opinion at a major cancer center as soon as possible may provide more options and support.
