How to treat bladder issues with pelvic floor exercises?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hello doctor,

I am 50. Last year, I felt a sudden urge to pass urine many times, even at night, three to four times. Sometimes I cannot hold it, and it leaks before reaching the bathroom. It is embarrassing in public. I have no sugar problem, and the ultrasound is normal.

Tried reducing coffee, but still the same. I heard about bladder training, pelvic floor exercises, and some medicines. Which works best? Are injections into the bladder safe? I just want a normal life without running to the toilet always.

Please help.

Thank you.

Hello,

Welcome to icliniq.com.

I understand your concern.

I have reviewed your concerns regarding the urgent need to pass urine, and I am sorry for the discomfort you are experiencing.

At your age of 50, such symptoms require further evaluation. You mentioned that your tests and ultrasound results were normal. Could you please provide a photo of the reports so I can review your case in more detail? I suspect a possible prostate issue, but confirmatory tests are needed.

In these cases, it is important to rule out organic causes, including balanitis if you are not circumcised. If no organic cause is found, the symptoms may be related to weakened pelvic floor muscles.

Pelvic floor exercises can help improve urinary symptoms. Kegel exercises, in particular, can be very beneficial. Although you mentioned that you are avoiding urinary irritants such as coffee, you should also avoid alcohol, packaged foods, and acidic foods to help reduce symptoms.

Medication can be prescribed depending on the test results. Please provide the reports so I can suggest the most appropriate treatment for your condition.

Treatment is always based on test findings. In addition to dietary modifications, you should increase your water intake to about three liters per day. It is also advisable to measure the amount of urine you pass to help evaluate kidney function.

You may take one teaspoon of baking soda at bedtime and observe whether it provides any relief. I look forward to receiving the requested reports.

I hope this helps.

Kindly follow up if you have more concerns.

Thank you.
