My mother has high BP. Is urgent medical care needed?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hello doctor,

My mother's age is 54. Her medical reports are unsatisfying. Can you please let me know if she needs urgent medical treatment or if we can wait? Her BP is high, and she is taking Telma.

Please help.

ello,

Welcome to icliniq.com.

Thank you for sharing your mother's laboratory report with us (the attachments were removed to protect the patient's identity). This report is for a cardiac enzyme called troponin I, and her troponin I value is 23.114 pg/mL.

The troponin test is performed to detect myocardial injury (heart muscle damage) in cases of chest pain or suspected myocardial infarction (heart attack). In such events, the troponin value would typically be much greater than the normal upper limit.

A proper diagnosis requires serial troponin tests and an ECG (electrocardiography) analysis. Only after reviewing these together can a definitive diagnosis be made. The laboratory result you shared is within normal limits. If her ECG is fine and she is asymptomatic (not experiencing symptoms), she should continue her current medical management.

However, if she is experiencing chest pain and has corresponding ECG changes, I would strongly suggest speaking to a cardiologist in person. This is crucial for proper diagnosis and treatment.

If necessary, the cardiologist may recommend tests for risk stratification, such as a myocardial perfusion scan, CT (computed tomography), coronary angiogram, or invasive coronary angiogram to diagnose your mother's condition promptly and plan treatment accordingly.

I hope this information is helpful.

Please do not hesitate to contact me if you need any further information; I would be happy to help you.

Regards.

Patient's Query

Hello doctor,

Thank you for your reply.

She has no pain in her chest, but she is feeling tired and low.

Please help.

Answered by Dr. Wajahat

Hello,

Welcome back to icliniq.com.

This is very reassuring, as she is not having any health issues. She can continue her medical management, and meanwhile, can book an appointment with a cardiologist for further planning and treatment.

The Troponin I results you shared were normal. I wish her good health.

Please let me know if you want to know anything else, and I would be happy to answer.

Regards.
