What could be done to manage impulsiveness and irritability?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hi doctor,

I am a 16-year-old female, weigh 118.3 pounds, and am 5 feet, 5 inches. I am not on any medications at the moment. I might have BPD because I have antisocial behavior at times. I do have compulsive behavior; I am very impulsive and can get irritable quickly, I engage in risk-taking behaviors, social isolation, self-destructive behavior, and am easily angered. I do have anxiety. I have a lot of guilt and feel lonely, and my mood swings can switch from being happy to being mad and crying right back to being happy within a matter of minutes. I do have depression, ADHD, and PTSD. I was on Fluoxetine for my depression, Methylphenidate for my ADHD, and Melatonin for my sleep. Please help.

Thank you.

Hi,

Welcome to icliniq.com.

I can understand your concern.

Thank you for taking help from a psychiatrist. You may have a borderline personality disorder. It is a type of personality. You may have a chronic feeling of emptiness, emotional outbursts, overwhelming behavior, and self-harming behavior. You might feel that no one understands you in this world. You may have difficulty maintaining relationships for a long time. You may have frequent job-related issues. You may have anger outbursts. There is non-pharmacological management for it. I suggest you try to follow the guidelines below to deal with it. It will help you.

1. You should exercise for 30 to 45 minutes daily, like at least going for a walk.

2. You should do meditation for 20 minutes daily. Sit in one comfortable position and try to concentrate on your breathing. Do not stop or force yourself to stop your thoughts. Just let it be.

3. You should eat healthy, home-cooked foods and avoid junk food.

4. You should spend time doing creative hobbies like reading good books, writing journals, painting, drawing, and learning new skills, languages, or musical instruments.

5. You should get enough sleep, at least seven to eight hours a day.

6. You must avoid tobacco, alcohol, or any other substance abuse.

Live your life, do not just spend it. Be bold and confident. I suggest restarting Fluoxetine 20 mg daily in the morning after breakfast. It will surely help you. Consult your doctor and take the medicine accordingly.

I hope that you get your answer.

Please let me know if you want some help.

Thank you.
