How can breast cancer patients manage bone health?

Hello doctor,

I have stage 2 breast cancer and osteoporosis. My bones are deteriorating rapidly. I have fractured my wrist just from opening a jar. The hormone therapy is worsening my bone loss, but I cannot stop it because the cancer might return. I have already had two vertebral fractures this year. I tried taking calcium supplements, but they interfere with my heart medications.

Are there any solutions? Is my bone health deteriorating because of the breast cancer? Is there any way to manage this?

The symptoms may be due to rheumatoid arthritis.

Hormonal therapy for breast cancer contributes to bone loss, increasing the risk of osteoporosis and fractures. Tamoxifen, for example, blocks the effects of estrogen on breast cancer cells. Estrogen is vital for bone metabolism, as it helps promote the activity of bone-forming cells.

The following options may help:

  1. Bisphosphonates or RANKL inhibitors strengthen bones and reduce the risk of breast cancer spreading. These include Alendronate, Zoledronic acid, Risedronate, and Ibandronate, which can be administered as tablets or via intravenous injection.

  2. Denosumab reduces the risk of fractures and slows bone loss in osteoporosis. It is given as an injection twice a year.

  3. Raloxifene and teriparatide for treating osteoporosis.

  4. Vitamin D supplements.

  5. Weight-bearing exercises, such as walking, help improve bone health.

  6. Avoid smoking and limit alcohol intake.

Management should be coordinated between your oncologist, endocrinologist, and primary care physician.
