Hello doctor,
I am a 30-year-old female with a history of fever of unknown origin (FUO) for many years. Recently, I found a lump in my left breast, and an ultrasound showed a lesion that is probably a debris-filled duct, classified as Breast Imaging-Reporting and Data System (BIRADS). I also have diffuse lymphadenopathy in my left collarbone, neck, submental area, antecubital space, and bilateral groin. I had a complete blood count (CBC), sedimentation rate (sed rate), and mononucleosis test (mono) done yesterday, which only showed a slight elevation in immature granulocytes (0.7 absolute). My primary care physician (PCP) is taking a watch-and-wait approach for all of my symptoms, but the longer I wait, the more signs and symptoms seem to appear. Any idea what might be going on?

I can understand your concern.
Your blood report shows some immature granulocytes and the enlarged lymph nodes suggest a benign, possibly infective cause. If needed, an antibiotic course can be prescribed. However, if the lymph nodes do not resolve and abnormal findings persist in repeat blood tests, further evaluation with clinical correlation is necessary. A biopsy of the lesion and computed tomography scans may be needed for a more thorough workup.
