How do I manage my breast milk discharge and delayed periods?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hello doctor,

My left breast is producing milk, I have stomach pain, and my periods are delayed this month.

Please tell me, why is this happening?

Kindly help.

Hello,

Welcome to icliniq.com.

I have read your query and can understand your concern.

You are noticing milk discharge from the left breast, delayed periods, and stomach pain. This combination suggests a possible hormonal imbalance, often associated with elevated prolactin levels. Although it can sometimes be related to stress or weight-related cycle disturbances, the presence of breast discharge requires careful evaluation.

I would recommend the following investigations to be done:

  • Urine or blood pregnancy test (to rule out pregnancy).

  • Serum prolactin level.

  • Thyroid profile (thyroid stimulating hormone, triiodothyronine, thyroxine).

  • Ultrasound of the abdomen and pelvis (to check uterus and ovaries).

  • If breast discharge persists: Mammogram or breast ultrasound.

I would suggest the following treatment plan:

  • Do not start any medicines before completing the investigations.

  • Complete the blood work and ultrasound.

  • If prolactin levels are elevated, an endocrinologist or gynecologist will prescribe appropriate medicines to lower them.

  • If the pregnancy test is positive, that explains the delayed cycle and symptoms.

  • For stomach pain: Short-term use of Antacid (proton pump inhibitors such as Omeprazole – gastric acid suppressor class) or Spasmolytic agents (Dicyclomine- anticholinergic/antispasmodic class) may provide relief, but the underlying cause must be confirmed before continuing.

I would suggest the following preventive measures:

  • Maintain a healthy weight.

  • Reduce stress.

  • Limit caffeine intake.

  • Ensure regular sleep for better hormonal balance.

I hope this helps.

Thank you.
