Why is my fiancée having breast pain and irregular periods?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hello doctor,

My fiancée is experiencing continuous breast pain due to irregular periods, such as only spotting instead of normal menstrual bleeding. Doctors have mentioned that this can be normal in some women, but it is causing significant discomfort and frustration. Please suggest what she can do to relieve the breast pain.

Kindly advise.

Answered by Dr. Ali Osman

Hello,

Welcome to icliniq.com.

I understand the concern.

Your fiancée’s breast pain related to irregular periods and spotting is most likely cyclical mastalgia, which is common in women who experience hormonal fluctuations. This occurs due to changes in estrogen and progesterone levels during irregular menstrual cycles.

I would suggest the following measures:

  • Wearing a well-fitted and supportive brassiere throughout the day can reduce discomfort.

  • Limiting caffeine and high-fat foods may worsen breast tenderness in some women.

  • Over-the-counter nonsteroidal anti-inflammatory drugs (NSAIDs), such as Ibuprofen or Acetaminophen, can be taken during painful days as per the recommended dose.

  • Applying warm or cold packs may help soothe discomfort.

  • Regular physical activity can support hormonal balance.

  • Monitoring when spotting or pain occurs may help in planning symptom relief.

If the breast pain is persistent, severe, or associated with a lump, nipple discharge, skin redness, or swelling, she should consult a gynecologist or breast specialist for proper evaluation to rule out other possible causes.

Hormonal treatments, such as combined oral contraceptive pills containing Ethinyl estradiol and Levonorgestrel, or progesterone therapy like Medroxyprogesterone acetate, may be considered if menstrual cycles remain irregular and symptoms are troublesome. However, these should only be used under medical supervision.

I hope you are satisfied with my answer. For further queries, you can consult me at iCliniq.

Thank you.
