Hello doctor,

What are these microscopic floating bubble-like particles floating on the top of my urine? They are difficult to notice, but under bright light, they float around on the surface, too little to tell if they are bubbles or not, yet they do not vanish like typical bubbles. They even resemble little globules of something. I have had urinary problems for months. I experienced redness on the glans penis and urethra entrance, which seems to have cleared up with antifungal cream and prescription. I had a urine test 15 days ago, and the results were positive for E. coli and Enterococcus faecalis, both at 100.000 kol per ml. My doctor stated that this was not concerning, but I was still prescribed Amoxicillin three times a day for seven days and Fluconazole two tablets once a week for three weeks. I finished Fluconazole today by taking the final two tablets. I started taking Amoxicillin. The redness from the Fluconazole is nearly gone, and I believe I feel better overall, but I am not sure whether the painful feeling after peeing is completely gone. So, to clarify, are the floating particles normal? I do not recall seeing them in the previous six months. And is 100.000 kol/ml of E.coli and Enterococcus faecalis a UTI infection? Could there be something else I am overlooking that could give me all these issues of redness, burning pee, particles, and yeast infections? I can try to get a photo next time I see the particles and add them here.

Before I begin, I would like to request that you send a picture of your urine (ideally in a container) so that I can determine what the floating items are visible. Could you possibly assist me with the following details? Do you have diabetes? Do you have any kidney-related issues? Would you describe your urine as frothy? Are the particles always floating, or do they eventually evaporate or drop to the bottom? Did they give you a urine routine and a culture test? Pus flakes and frothy urine owe to renal illness, and fat globules are all possibilities. Concerning the glans inflammation: Do you have a circumcision? If not, does the skin glide readily back and forth, or is it stuck? How long have you been dealing with this problem? Is this the first time this has happened, or has it happened before? Do you have to strain to pass urine, or do you get the feeling of an incomplete bladder emptying or a sense of urgency? Did your doctor diagnose you with balanoposthitis? Did your doctor also screen you for diabetes? I would also like a picture of the penis to look at. Also, share any test results that you have completed and have on hand. A positive urine culture with a colony count of a hundred thousand is serious and necessitates therapy, which your doctor has already prescribed. Hello doctor,

It is difficult to photograph the urine, so I will try again tomorrow.I posted a photo this time, which is poor, it is within the red circles, and the darker bits are from the cup. When the redness surrounding my penis was at its worst, I snapped some shots. This happened a few months ago, but it happened multiple times after I stopped using antifungal treatments. Please see the photographs attached. I changed physicians due to a relocation, and my new doctors did not say anything. Let me attempt to respond to your queries. I am not diabetic, and I underwent blood and urine testing.

Would you describe your urine is frothy?

It is not mostly clear with a few spots of foam that settle quickly and do not stay around.

Are the particles always floating, or do they eventually evaporate or drop to the bottom?

I just saw it from the top, but the particles do not appear to be settling to the bottom. It remains on top, and it appears to be little globules or a film layer of some type, but if I lightly touch it, it goes away.

Did they give you a urine routine and a culture test?

I have experienced UTI since I was 18 years old. Around two years ago, I had a cystoscopy. They detected nothing, despite many urine tests for UTI that yielded no results. I also examined how effectively I emptied my bladder, which was typical. Pus flakes, frothy urine owing to renal illness, and fat globules are all possibilities. Also, I think that began after I began taking isotretinoin for my skin problems. Or, at least, it was the first thing I noticed after beginning this medication six months ago, however, I stopped taking the medication three months ago.

Coming to the inflammation on your glans:

Are you circumcised? If not, does the skin move back and forth easily or is it stuck?

No, I am uncircumcised, it felt a little more stuck, but I was still able to pull it back.

How long have you been dealing with this problem?

This has been troubling me for about six to seven months. I became aware after having sex with my wife. During intercourse, I felt a burning sensation and then a strong desire to urinate, followed by a few times of urination. I wait a few minutes and pee again, then repeat for another fifteen to twenty minutes. There were quite severe UTI symptoms, but my dipstick was negative, so I phoned my doctor, who felt the penis was red and said it may be yeast because my dipstick was negative. Then I treated it with antifungal creams on and off for a month or two, then went to see my dermatologist, who told me to put steroid cream on and was not very helpful. I returned to my doctor. They performed STI swab testing, and all were negative. The urine sample was clean, although I had to conduct the test on a day when I didn't have urine in my bladder for very long if that counts. Symptoms subsided for a while, but when I started another cycle of isotretinoin, they reappeared. Because of this, I stopped using isotretinoin and have not used it since. The redness would appear in a few days. My internal symptoms are a searing sensation that is difficult to identify and describe accurately. I feel it most when I urinate before going to bed and then lie down on my back. It is like, coming from the urethra or perineum, or maybe it is radiating out. I am not sure how to describe it. But I primarily feel it in bed. I also have sex troubles, ever since my UTI began, I have had discomfort in the perineum after ejaculating, it is a heavy, unpleasant feeling that worsens when I get up. It generally begins quickly after intercourse and worsens after about 20 minutes. Urinating may relieve discomfort if I urinate quickly after sex, which has led to certain unhealthy behaviors such as retaining urine or waiting to urinate after sex. No doctor could tell me what was causing all of this, but I know it is all related.

Is this the first time this has happened, or has it happened before?

I believe I tried it once previously, but I am not sure, it was on a trip when I detected redness around my penis glans and had to go to the hospital, where they discovered I had a UTI.

Do you have to strain to urinate, or do you get the sensation of an incomplete bladder emptying or a sense of urgency?

I generally feel like it is finished. But if I try to urinate and my bladder is not full, I get this cramping sensation. It also happens if I squat down to urinate. I also get frequent intense desires to urinate that come out of nowhere, even though my bladder is not overflowing.

Did your doctor diagnose you with balanoposthitis?

They did not say anything.

Did your doctor also screen you for diabetes?

Yes, he performed all of the above tests and informed me that I do not have diabetes.

A positive urine culture with a colony count of 100k is serious and necessitates therapy (which your doctor has already prescribed). The only weird thing is that when I had UTI in the past, I have generally experienced severe symptoms, and this time I did not experience the same intense symptoms. The photo of your penis from today appears to be rather normal to me (attachments removed to protect the patient's identity). I agree that it seems a little inflamed in some of the pictures. For the time being, I would leave things alone and avoid using isotretinoin until necessary. I have a strong suspicion that you are suffering from chronic prostatitis. It manifests as these persistent problems and influences sex life, and bladder and prostate health overall. I honestly am unable to determine from the photographs what the floating material in your urine is. Perhaps a urine test can help determine what it is. I will give you some general and medical advice to follow over the following six months. I believe it is prostatitis, and with this medication, there is a significant chance of recovery. I suggest you follow these instructions: drink 3 liters of water each day, masturbate or have sex thrice a week, and avoid sitting for long hours of time or riding a bicycle. I also recommend that you take the following medication: tablet Cranpac D (Dietary supplement) twice daily. Econorm 250 mg (Probiotic supplement) twice daily, capsule Tamsulosin (Alpha blocker) 0.4 mg once daily for three to six months, tablet Chymoral Forte (Trypsin and Chymotrypsin) twice daily once a month, capsule Doxy (Doxycycline) 100 mg twice daily once a month, tablet Ciplox (ciprofloxacin) 500 mg twice daily once a month. Hello doctor.

As you can see from the images, I was feeling better at first. I completed Amoxicillin three times a day until last week after finishing Fluconazole the week before. Now I notice that the tip of my penis is becoming a little itchy. Otherwise, I was fine until today, when I masturbated I felt a mild burning sensation. Twenty minutes later, I had a shower and discovered that the area surrounding the urethra entrance was quite red. I have included a picture of how it now appears, it is already quite red and inflamed compared to just last week. It was less red before masturbating, and masturbating aggravates the condition further even when I am extremely careful. I cannot comprehend this, and it cannot be right. What exactly is going on, and why is this happening? Is this yeast once more? Why would I get this again, and why does it seem to worsen with sexual activity? Is it due to the antibiotics I took? As I already stated, I believe it is chronic prostatitis. Along with the irritation of your urethra, there appears to be an aspect of urethritis. I did list the treatment for you last time, which will also assist with the urethritis. Did you have an opportunity to begin any of it? Because you would have experienced cheesy discharge from and around your penis if you had a yeast infection, I would not advise you to use Fluconazole. I would recommend that you get a sperm culture done, and also begin taking the medication and recommendations I suggested in our last conversation. If you cannot find it, let me know and I will repost it. You should be aware that therapy for chronic prostatitis might take up to six months. There is also the possibility of relapse in the future.
