Hello doctor,
I have been experiencing health anxiety regarding a concern, and would appreciate your guidance. I have noticed multiple tiny bumps around the head of my penis. They are not painful, itchy, or associated with any discharge.
I have had them since I was around 18 years of age, and there have been no significant changes in size or number. Could you please let me know if this is something harmful or a normal variation? I am worried whether this could indicate a serious condition.

Based on the photo you shared and your history, these tiny bumps have been present since around age 18, are not painful or itchy, and have not changed significantly. This most closely appears to be pearly penile papules (PPP).
These are very common and completely normal anatomical variations, frequently seen in males during or after puberty. They are not an infection, not a sexually transmitted disease, not related to sexual activity, and they are not cancerous.
They typically look like small, smooth, skin-colored or whitish dome-shaped bumps arranged in one or two neat rows around the rim (corona) of the penis head, which matches your description. Importantly, they do not spread, do not develop into anything dangerous, and do not affect sexual function or fertility in any way.
Studies show that about 20 to 30 percent of men have them. Many simply do not talk about it, which is why it can cause anxiety when someone first becomes aware of it.
The fact that they have remained stable for years, with no pain, itching, discharge, ulceration, or rapid growth, is very reassuring and strongly indicates that this is a harmless and benign finding.
They are different from genital warts, which usually appear irregular, rough, and cauliflower-like, and tend to grow or spread. Your bumps appear smooth, uniform, and symmetrically arranged, classic features of pearly penile papules.
No treatment is required because they are harmless. Removal is only considered for cosmetic reasons and is not medically necessary.
Based on what you have shown and described, your penis appears healthy. There are no signs of infection, STD (sexually transmitted diseases), or anything dangerous.
Your health anxiety around this is completely understandable. But medically speaking, this is a normal variation of anatomy.
