Patient's Query

Hi doctor,

I have burning sensation and discomfort in feet, toes, hands, arms, and back of my thighs. I have high cholesterol. I have a uric acid of 8.6 mg/dL. I had an H.pylori infection, and I had completed treatment for it. I have erosive esophagitis and gastritis. I have chronic bipolar depression and anxiety. I am taking Clonazepam, Zincovit, and Montair FX. My doctor advised me to change my lifestyle and food habits. I am worried about the burning sensation. It has got increased in the last two weeks. I am in the prediabetic stage. Please help me.

Hi,

Welcome to icliniq.com.

Your symptoms are related to peripheral neuropathy. High uric acid levels can trigger the clinical situation. I would recommend consulting a neurologist and perform nerve conduction studies to investigate for possible neuropathy. Gabapentin or Pregabalin can help to improve your situation. You should discuss with your doctor on the above issues.

Patient's Query

Thank you doctor,

Do these symptoms are caused due to bipolar depression and anxiety disorder? I am losing my mind. I have many physical issues coupled with mental problems. I am tensed. Can I get fine after having lifestyle changes, exercise, and meditation? Please advise.

Hi,

Welcome back to icliniq.com.

I can understand your concern. Anxiety and depression may exacerbate these symptoms. Your uric acid and fasting glucose levels (prediabetic) are the main causes for having these underlying complaints. A healthy lifestyle and physical activity can help to improve your situation. But I do not think that they will make a miracle. Start taking Gabapentin or Pregabalin. It will help you to improve your situation and also helps to fight against anxiety. Try to follow a healthy lifestyle and closely monitor your fasting glucose and uric acid plasma levels.

Patient's Query

Thank you doctor,

Can I take Gabapentin or Pregabalin? Do these are long-term medications?

Hi,

Welcome back to icliniq.com.

Yes, they are long-term medications, and it is well tolerated.

Patient's Query

Thank you doctor,

My psychiatrist has given me Valproic acid to treat my bipolar depression. Does it affect my gastritis and ulcerative esophagitis? Please advice.

Hi,

Welcome back to icliniq.com.

Stop taking Valproic acid. It has a lot of adverse effects in your situation. Take Pregabalin or Gabapentin, which are tolerated better and can help improve bipolar disorder too.

Patient's Query

Thank you doctor,

Please explain about the dosage of the medication. Should I have to visit a neurologist soon? Can I take mastic gum and quasinoids? I also have discomfort and pain in stomach that occurred during H.pylori infection.

Hi,

Welcome back to icliniq.com.

These drugs are quite similar and should not be taken together. If you choose to start Gabapentin, I recommend starting with 300 mg (one tablet) daily and increase it slowly up to 900 mg per day. If you decide to begin Pregabalin, I would recommend starting with 75 mg (one tablet) and increase the dose to 150 mg daily. Regarding mastic gum and quasinoids, I have no experience in using them. Take another course of treatment for H. Pylori (Helicobacter pylori) infection.

Patient's Query

Thank you doctor,

Thank you so much.

Hi,

Welcome back to icliniq.com.

You are welcome.

Patient's Query

Hi doctor,

I have a uric acid of 8.6. I have been given Mycolon-C. I have read that Chlordiazepoxide can increase uric acid. My nephrologist asked me to talk to my gastroenterologist. The doctor told me that there is nothing to worry. Please suggest.

Hi,

Welcome back to icliniq.com.

I can understand your concern. Chlordiazepoxide does not alter the uric acid plasma levels. It is used to improve patients with high uric acid symptoms (pain related to insomnia).
