Is a heart rate of 54 bpm with chest pain serious?
Patient's Query
Hello doctor,
I am 36 years old, my height is five feet seven inches, and my weight is about 161 pounds. I have been experiencing mild pain on the left side of my chest (heart side).
I had an ECG, which showed a heart rate of 54 beats per minute. At times, I also experience difficulty in breathing.
Kindly help.
Hello,
Welcome to icliniq.com.
I understand your concern.
Thank you for sharing the details. Chest pain associated with a slow heart rate (54 beats per minute) and occasional breathing difficulty requires careful evaluation. A heart rate of 54 per minute can be normal in young, physically active individuals due to increased vagal tone.
However, if you are not very physically active and are experiencing chest discomfort or shortness of breath, this should not be ignored.
The aim is to determine whether the symptoms are cardiac in origin or due to non-cardiac causes such as gastritis, musculoskeletal pain, or anxiety.
The possible causes for your symptoms include conditions affecting the heart rhythm, gastrointestinal system, musculoskeletal structures, or stress-related factors.
-
Sinus bradycardia (slow heart rate; may be physiological or related to increased vagal tone).
-
Anxiety- or stress-related chest discomfort.
-
Gastritis or gastroesophageal reflux disease (GERD).
-
Early conduction system abnormality or coronary artery spasm (rare, but possible at this age)
The following investigations are advised to identify the underlying cause and to rule out significant cardiac disease. These include:
-
Repeat ECG (echocardiogram), preferably during an episode of chest pain.
-
Echocardiogram to assess heart structure and pumping function.
-
Treadmill stress test (TMT) to rule out myocardial ischemia.
-
Blood investigations: complete blood count (CBC), fasting blood sugar, lipid profile, and thyroid-stimulating hormone (TSH).
Based on the available information, the most likely diagnosis is benign sinus bradycardia associated with non-cardiac chest pain; however, cardiac causes such as coronary artery disease must be ruled out due to the presence of chest discomfort and breathing difficulty.
The management approach focuses on:
-
Avoid heavy physical exertion until evaluation is complete.
-
Lifestyle modifications: avoid excess caffeine, alcohol, and smoking.
-
Pantoprazole 40 milligrams once daily before breakfast for five days (if acid-related pain suspected).
-
Immediate medical attention if giddiness, fainting, sweating, worsening chest pain, or severe breathlessness occurs.
Follow-up is important to review investigation results and guide further management. You should share the ECG report and blood pressure readings if available, and undergo echocardiography and treadmill stress testing as advised.
Preventive strategies help reduce symptom recurrence and improve overall cardiovascular health. They include:
-
Regular exercise after medical clearance
-
A balanced diet with reduced oily and fried foods.
-
Adequate sleep and stress management through relaxation techniques
I hope this helps you.
Thank you.
